💎 NICU Pearls | NEC 2 of 4: Barrier Failure & Oxygen Injury

🍼A healthy mature gut digests and absorbs nutrients through a strong barrier. The preterm gut is different: thinner mucus layer, weaker tight junctions, immature immune defense, and altered motility.

🦠In the presence of pathogenic bacteria, pro-inflammatory signals, and toxin production, the barrier weakens further.

⛔️Then oxygen turns against the gut: fluctuating blood flow and high oxygen exposure generate reactive oxygen species (ROS) that damage intestinal cells, mitochondria, proteins, lipids, and DNA.

💎 NICU Pearls | NEC 1 of 4: The Fragile Start

👶🏽In utero, the gut develops in a sterile, steady environment with continuous nutrients, stable blood flow and low oxygen enabling optimal maturation.

⛔️Premature birth interrupts it all causing immature gut barrier and immune system, altered blood flow regulation, with exposure to high oxygen, medications, and microbes, and high risk for oxidative stress.

🍼How to protect the baby from birth: early minimal enteral (trophic) feeds with breast milk mimic the in utero environment, stimulate gut maturation and motility, and reduce the risk of NEC, along with judicious oxygen use.

It’s been 19 years, and I still miss you
I have accepted you are gone
I feel grateful for the brief yet most memorable experience of my life,
yet a feeling of loss lingers like a wisp from a feather in fleeting moments🤍

🎉Kamsi began her project as a 2nd-year undergraduate (now a 2026 graduate), and received a competitive University of Rochester Office of Undergraduate Research Schwartz Discover Grant for her travel to Nigeria in summer 2024. Kamsi received two more UR Office of undergraduate research grants to support her 2025 poster presentations at APHA and Student National Medical Association AMEC conferences.

Love is God among us.☀️❤️‍🔥

�❤️ Love was the theme for Sunday readings. The gospel shares that the greatest commandment is to love God above all, and then your neighbor as yourself.

❤️As a lector, I did the first reading from Exodus 22:20-26 and universal prayer. We are called to show compassion to our neighbors. Do not oppress aliens to a new land, widows, or orphans (those who feel alone or suffer physical or spiritual loss), return one’s cloak as promised, and do not overcharge interests to borrowers (show integrity and kindness).

💘If not, God says of our neighbor, “If he cries out to me, I will hear him; for I am compassionate” and “I will kill you with the sword.” God loves us all but will always seek justice for the oppressed, suffering, and vulnerable.

☀️Our neighbor is not only our friend or family but a stranger in need, co-worker or those who look different from us.

💘One may ask why God allows suffering if He is compassionate. Our misuse of our free will causes sin and suffering to us or others, not God. Jesus reconciles us to God’s love by suffering for us on the cross. We are closest to Jesus when we suffer.

💘To suffer is to love. Love is sacrifice, and demands giving up something of ourselves for the well-being of another. Love makes the burden of suffering light.

💘Do not be afraid. You are never alone when you suffer. Suffering is the sweet embrace of God’s love. Pain during suffering is a sign that Jesus has hugged you so close to Him on the cross that you can feel the piercing of the thorns on His head.

☀️During suffering, keep your heart and eyes open for signs of God’s presence as He infuses love in your soul and transforms you. It could be the sunlight reflecting on a window, an unexpected phone call, tears from our heart overflowing with overwhelming love from God’s presence, a comforting thought, a sudden feeling of warmth, or someone showing you unexpected grace.

☀️Then, allow God to use you to bless someone else who may not recognize Jesus in their own suffering - with a kind word, a hug, warm gesture, or a listening ear.

🧠Know the risks - extreme prematurity, high or fluctuating oxygen, BP instability, anemia, transfusion, infection, fluid overload, abnormal CO₂, pain and stress - then mitigate them.

💎Pearls:
📍Judicious oxygen with bi-phasic targets: SpO₂ 85–92% days 1–30, then 92–97% from day 31

📍Stable blood pressure, CO₂, and temperature; prevent/treat anemia; avoid fluid overload and high FiO₂

📍 Serial cranial ultrasounds in high-risk infants - early detection improves outcomes

📍Many preterm infants go on to thrive with early detection and supportive care
