INTRAMUSCULAR INJECTIONS

THERE IS NO SUBSTITUTE FOR REAL TRAINING.

Medications such as epinephrine, midazolam, ketamine, and naloxone (Narcan) can be given by IM injection in emergency settings.

IM injections deposit medication into muscle tissue, where it is absorbed through nearby blood vessels into the bloodstream. Absorption depends partly on blood flow to that muscle. Poor perfusion can delay the medication’s onset and make its effects less predictable.

22–25 gauge needles, typically 1–1.5 inches long, are commonly used for adult IM injections. Choose the needle based on the medication, injection site, and patient’s size: it must reach the muscle through the overlying fat. Shorter needles may be appropriate for some pediatric IM injections, as well as subcutaneous or intradermal injections in adults.

If the intended site is injured, choose another site. Avoid wounds, burns, significant bruising or hematomas, swelling, infection, and muscle directly affected by trauma. Choose intact tissue with adequate muscle mass.

Patients in shock or with poor perfusion can make IM absorption delayed or unreliable. In an unstable trauma patient, favor IV or IO administration when appropriate for the medication

the new Mochtech Tibia IO Attachment — designed to help students progress from learning the landmarks to confidently performing the skill.
The attachment features a replaceable bone chip, allowing for multiple drilling repetitions without replacing the entire trainer. Students can also push fluids through the device, adding another layer of realism to IO training.
Training can begin with clear/translucent skin, allowing students to visualize the anatomy and confirm proper placement. As their muscle memory and confidence improve, they can transition to non-translucent skin tones, removing the visual aid and increasing the difficulty.

Conventional IV Trainers may look the part but they don’t teach difficult cancelations when it comes to loose skin, rolling veins, difficult angles and other variables.
