BTS on what we as physicians deal with when trying to get meds approved for our patients.
I deeply respect my family medicine and pharmacist colleagues, but unless the medical director has treated the same type of conditions or prescribed the medications/performed the procedure you're trying to get covered they are not "peers".

In California, the rules are stricter for what constitutes a "peer to peer". Hopefully, with the help of Assemblymember @asmdawnaddis and AB 510, this obvious barrier to care will become officially what we all know it to be...criminal.

In this circumstance, the patient's employer was able to get the med approved by threatening to drop the insurance carrier for their company.
This is what we need to do...real consequences for these companies who try to practice medicine without a license.

This is such an egregious overreach into patient care that is happening behind the scenes. Your physicians and APPs are dealing with this sort of thing instead of focusing on patient care.

Some Dermpath 101 for you! 🔬

You can't trust a negative "margin" on a biopsy. Clinical margins on excisions will include millimeters to centimeters of normal appearing skin around the area of cancer, but routine biopsies do not.
We also only see 0.2 to 1% of the specimen under the microscope.
