Women fought to get their menopause symptoms taken seriously. Now they’re fighting to get their prescriptions filled. Tell me how that makes sense… 🧐 You finally sleep. Your hot flashes improve. You start feeling like yourself again. Then, “sorry, it’s backordered.” The estradiol patch and progesterone shortages aren’t just pharmacy problems — they affect women trying to work, maintain relationships, and get through the day without feeling miserable. “Call another pharmacy” is not a care plan, but here’s where I want to be clear: advocating for hormone replacement therapy does not mean pretending hormones fix everything. We spent years making women afraid of HRT. Now parts of social media have swung to the other extreme, where every symptom supposedly needs more estrogen. Neither extreme is truly helpful to women. Hormone therapy can help the right patient, but it has risks and benefits. It isn’t mandatory, and it doesn’t replace evaluating what else might be going on. You deserve to be heard without being sold a miracle. You deserve options without being pressured. And when your medication is unavailable, you deserve help finding an alternative plan. If you’re struggling to fill your prescription, contact your pharmacist and prescriber before changing treatment and ask them what alternatives are appropriate for YOU. Does hormone therapy help the brain or hurt it? I get asked this all the time. A study recently published in Neurology, mentioned here by @rachelfrankenthal - went beyond memory tests and researchers examined Alzheimer’s changes in the brain at autopsy. Women who reported using estrogen-only hormone therapy had lower odds of Alzheimer’s pathology than women who reported no hormone therapy use. They also had lower odds of a dementia diagnosis. That’s promising, but it is not proof that estrogen prevents dementia. This was an observational study, and its estrogen-only findings cannot be applied to every hormone therapy regimen. A separate analysis of Medicare records for 10 million women also found an association between estrogen-only therapy after age 65 and lower dementia risk. So what does this mean? 📖 Women deserve a more precise conversation than “hormones cause dementia” or “hormones prevent dementia.” The type of therapy, your health history, and the reason you’re considering it matter and hormone replacement therapy is not currently recommended solely to prevent dementia. Your doctor can be wrong and the person on instagram or Tik tok telling you your doctor is wrong can also be wrong. Apparently we’ve reached the point where that qualifies as controversial… I’m an OBGYN who also trained in Integrative Medicine and hold a PhD in Mind Body Medicine — so no, I don’t believe conventional medicine deserves a free pass simply because it’s conventional. But I also don’t believe “holistic,” “natural,” “functional” or “alternative” should function as evidence so I went looking deeper into some of the practices I get asked about the most: Chiropractic. Naturopathy. Homeopathy. Applied Kinesiology. 💬 What were the original claims? 💬 What happens when those claims are put into controlled studies? 💬 Where does the evidence actually support their use? 💬 Where have we taken a small piece of evidence and stretched it into a much bigger promise? That last question is the important one because making claims like — “It may help back pain” and “it regulates your organs” are not the same thing. “Muscle strength can be clinically assessed” and “muscle testing can tell you which foods you’re allergic to” are not the same. Science doesn’t work by association and neither should your healthcare decisions. You finally found hormone replacement therapy that helps you feel like yourself and now the pharmacy says, “It’s on backorder.” Women should not have to become their own supply-chain managers to access menopause care and I’m glad to see another physician — karentangmd — bringing attention to the estradiol patch shortage and the barriers women face trying to fill their prescriptions. Increased demand is only part of this conversation — we also need to talk about reliable supply, pharmacy access, and what happens to the woman caught in the middle. Behind every delayed refill is someone wondering whether her hot flashes, night sweats, and disrupted sleep are about to return, so here’s what I want you to know: 💌 Trouble finding your estradiol transdermal patch or progesterone does not automatically mean you’re out of treatment options. Availability varies by manufacturer, dose, and pharmacy. 💌 Ask your pharmacist to check other available manufacturers, and contact your prescriber before you run out. A different formulation may be appropriate, but switching requires an individualized plan. 💌 If you have a uterus and use systemic estrogen, don’t simply leave out your prescribed progesterone because you can’t find it. Keep your clinician in the conversation and discuss maintaining protection for your uterine lining. Pinellas Commissioner Vince Nowicki told Creative Loafing Tampa Bay he respects Gualtieri and believes the sheriff was exhausted after a long round of questioning. Still, he disagrees with the sheriff’s argument that Flock cameras don’t track people. “It’s not physically tracking you, there’s no tracking device, but it is collecting people’s and vehicles’ movements, and it’s storing those movements in a chronological timestamped database to see where you go,” Nowicki told CL.
