Your GP probably mentioned hot flushes.

They probably didn't mention this.

Oestrogen plays a direct role in 
cognitive function.

Blood flow to the brain.
Neurotransmitter activity.
Neural connection maintenance.

When oestrogen declines, cognitive 
symptoms can appear — sometimes years 
before the hot flushes.

The brain fog isn't anxiety.
It isn't burnout.
It isn't you getting older.

It's oestrogen.

And here's the clinical part that matters:

Cognitive symptoms respond more slowly 
to HRT than vasomotor ones.

Hot flushes may improve in weeks.
Cognitive clarity can take 3-6 months.

If you're judging your HRT by hot flushes 
alone — you may stop before the cognitive 
benefit arrives.

Track the brain fog separately.

The difference between getting dismissed at your appointment and actually being heard usually isn't luck — it's one piece of paper. You know something's wrong. But in the room it comes out as "I just feel a bit off" — and that's easy to wave away. A feeling is easy to wave away. Evidence isn't. Nobody warns you about the week-3 dip on HRT.
You start treatment, feel a lift… then around week three the floor drops out, and you're convinced it isn't working.
Here's the reframe: that dip often isn't the treatment failing — it's your body adjusting to a signal it hasn't had in a long time. And it tends to come right before things settle.
Why it matters: around half of women stop HRT, often before it's had a real chance to work (Bunnewell et al., BJOG 2025 — a review of 32,000+ women). The week you're most likely to quit is often the week before it turns.
So if you're in the dip — don't quit blind. Note when it started, and give it the full window before you decide.

I did my Master's research on why people stop taking treatments that work. Then I looked at HRT — and it's one of the worst.

Around half of women stop taking HRT, often in the first few weeks — right before it would have started working. (A 2025 systematic review of 32,213 women found 51% discontinue.)

And here's the part that gets missed: it's rarely that the treatment failed. It's that no one explained the timeline — the dip, the wait, what "working" is even supposed to feel like.

That gap is the whole reason this account exists. I'm not here to reassure you — I'm here to show you the research and help you walk into your GP appointment prepared.

You're not sleeping badly because 
you're stressed.

You're sleeping badly because your 
oestrogen dropped.

These are different problems.

Perimenopausal sleep disruption 
has a specific pattern:

→ Waking consistently between 2-4am
→ Racing heart on waking
→ Feeling of heat
→ Cannot return to sleep

Not random. Not stress. A pattern.

Here's the mechanism:

Oestrogen regulates sleep two ways:

1. Core body temperature at night
When oestrogen drops — temperature 
regulation becomes erratic.
Sleep architecture breaks.

2. Serotonin and melatonin production
Both critical for sleep initiation 
and maintenance.
When oestrogen fluctuates — so do both.

You wake up because your sleep 
chemistry has changed.

Not because you have something to worry about.

HRT addresses both mechanisms directly.

Sleep hygiene addresses behaviour.
HRT addresses the chemistry.

If chemistry is the cause —
behaviour change alone won't fix it.

Track your sleep pattern before 
your next appointment.

Time of waking. How you feel. 
Whether there's heat.

That's clinical data.

Things perimenopause does that 
sound completely made up.

They're not.

→ Forgetting words mid-sentence
→ Ears ringing for no reason
→ Crying at an advert
→ Feeling claustrophobic in your skin
→ Waking at exactly 3am every night

All of this is the oestrogen.
None of it is you losing your mind.

What your GP sees in a 10-minute 
appointment.

What they can't possibly see.

These are not the same list.

WHAT THEY SEE:
→ How you present in the room today
→ Whether you seem distressed
→ Their last notes on your file
→ Your general history

WHAT THEY CAN'T SEE:
→ How you felt at 3am last Tuesday
→ Your symptom pattern over 8 weeks
→ Whether bad days cluster 
  around your cycle
→ Whether your compliance 
  has been consistent

None of that exists in the appointment 
unless you bring it.

Your GP is not the problem.

10 minutes with no pre-prepared data 
limits any clinician — not because 
they're bad at their job.

Because clinical decisions require 
information.

The gap between what your GP sees 
and what they need is structural.

The only person who can close it is you.

Not with a list of complaints.
With documented, structured data.
