✅ Diagnosis:

Lymphoma

Significant Findings:

CT examination demonstrates irregular, contrast-enhancing thickening of the right hemidiaphragm with internal hypodense areas. (Arrow) There is an infiltrative, contrast-enhancing process involving the right adrenal gland and retroperitoneum, extending perirenally (arrow) and along the renal vessels (Star), with encasement of the abdominal aorta (arrow), celiac
trunk, and superior mesenteric artery. Partial involvement of the descending duodenum (pars descendens) and transverse colon is noted.
Pathologically enlarged right para-aortic lymph nodes are present. (Star) There is associated subtle fat stranding and/or small fluid collections in the affected regions. A large right pleural effusion is present, with adjacent compressive atelectasis.

☢️ Imaging findings:

Ultrasound
• Large homogeneous hypoechoic retroperitoneal mass.
• Internal vascular flow demonstrated on color
Doppler imaging.
• Associated enlarged hypochoic lymph nodes may be present.

CT
• Bulky homogeneous soft-tissue mass within the retroperitoneum.
• Confluent para-aortic, aortocaval, mesenteric, and iliac lymphadenopathy.
• Encasement of major abdominal vessels without significant compression or thrombosis („sandwich sign“).
• Mild to moderate homogeneous contrast enhancement.
• Absence of significant calcification in untreated disease.
• Adjacent organs are displaced rather than invaded.

MRI
• Low to intermediate signal intensity on T1-weighted images.
• Intermediate to mildly increased signal intensity on T2-weighted images.
• Marked diffusion restriction on diffusion-weighted imaging due to high cellularity.
• Relatively homogeneous enhancement following gadolinium administration.

FDG-PET/CT
• Intense FDG uptake within the retroperitoneal mass and involved lymph nodes.
• Useful for disease staging and treatment response assessment.
• May reveal additional nodal and extranodal sites of involvement not evident on conventional imaging. by @radiologyplanet

Eagle syndrome refers to symptomatic elongation of the styloid process or calcified stylohyoid ligament. It is often bilateral. In most cases, the cause is unknown; however, the condition is sometimes associated with disorders causing heterotopic calcification such as abnormal calcium/phosphorus metabolism and chronic renal failure.
💪🏼 Cranial nerve impingement
Patients can have symptoms related to compression and irritation of cranial nerves in the region (cranial nerves V, VII, IX and X) such as:

🚩facial pain when turning the head
🚩dysphagia
🚩foreign body sensation
🚩pain on extending tongue
🚩change in voice
🚩sensation of hypersalivation
🚩tinnitus or otalgia
🚩On palpation of the styloid process tip, symptoms should ideally be exacerbated.

📝A 29-year-old pregnant woman presented with a 2-month
history of hyperemesis gravidarum, now experiencing
sudden confusion, vision blurring, and altered
consciousness over the past 5 days. This clinical scenario
points to Wernicke encephalopathy, an acute neurological
syndrome stemming from thiamine (vitamin B1)
deficiency. While it's often linked to chronic alcoholism, it
can also occur in conditions like malnutrition, postbariatric
surgery, hyperemesis gravidarum, and gastrointestinal
malignancies.
Clinically, Wernicke encephalopathy presents with a
classic triad: confusion, ataxia, and ophthalmoplegia.
☢️MRI☢️
findings typically show symmetric hyperintensities on
T2-weighted and T2 FLAIR images in the dorsomedial
thalami, mammillary bodies, and periaqueductal area,
along with mild to moderate water diffusion restriction in
these regions. Immediate thiamine administration is
crucial to correct the deficiency and prevent further
neurological damage.

✅ Diagnosis:

Polymyositis

Significant Findings:

Bilateral streaky areas of high T2/STIR signal intensity in quadriceps femoris muscle, adductor muscles, obdurator muscles, gracilis muscle and in the anterior lower leg muscles with no skin involvement.

☢️ Imaging findings:

MRI:

T2/STIR: affected regions during active disease have high signal intensity, reflecting muscle oedema. Muscle edema is generally seen in the proximal muscles in a bilateral symmetric distribution and most commonly involves the vastus musculature.

Other features, in the chronic phase, may include:

1. fatty infiltration of muscles.
2. muscle calcification.

💡Differential Diagnosis:

1. inclusion body myositis
-usually involving patients over 50 years.
2. dermatomyositis
-will have skin features.
3. anti-synthetase syndrome
-will have skin features.
4. traumatic myopathy
-usually, will not involve all the muscular compartments and will not present with a relatively symmetrical bilateral pattern.
5. collagen vascular diseases (e.g. systemic lupus erythematosus, Sjögren syndrome, and rheumatoid arthritis)
-has a more focal, sometimes nodular, myositis.
6. drug-induced myositis (e.g. statins, antiretroviral drugs (e.g. d-drugs) used in HIV treatment, checkpoint inhibitors)
-reversible after discontinuation of the medication.
7. muscular dystrophy (e.g. Duchenne)
-muscles evolve from an oedematous phase to become atrophic.
-there is no associated skin and subcutaneous oedema

Hoy realizamos la punción de esta lesión visualizada en la mamografía como una lesión nodular, que en principio parecía ser de aspecto sólido, ubicada en el cuadrante supero externo de la mama derecha. En la ecografía se visualizó como un conglomerado de quistes , dos de estos tenían una parte sólida en su interior, con flujo al Doppler Color, de las cuales se tomaron las muestras. taken in Hospital de Clinicas by @radiologyplanet
