When I was a sophomore in high school, my mom gave me a Forbes magazine with a list of careers projected to have the most growth by 2030. Physical therapy was #7 on the list.

At the time, I had no idea where that career would take me.

Fast forward a few years, after completing numerous traveling contracts, starting my own practice, and now supervising rehab at a retirement community, I’ve realized just how much I’ve grown to love this profession.

One of the greatest gifts of being a physical therapist isn’t just the ability to help people move better or recover from injury β€” it’s the relationships you build along the way. Getting to know my patients, hearing their stories, and being trusted to be a part of their journey is something I’ll never take for granted.

Last night, I had the opportunity to guest speak at an event for one of my patients, and it was one of those moments that made me pause and appreciate how far this career has taken me.

POV: your β€œpickleball elbow” won’t go away 🎾

Lateral elbow pain (tennis/pickleball elbow) isn’t a stretching problem β€” it’s a tendon load tolerance problem.
If you only rest, massage, or brace it, the tendon never rebuilds capacity.

Here’s a simple 3-step progression I use clinically:

1️⃣ Wrist Extension Isometrics (Pain modulation phase)
β€’ 30–45 sec holds
β€’ 4–5 sets
β€’ 1–2x/day
β€’ Pain ≀ 3/10

2️⃣ Wrist Extension Eccentrics (Tendon remodeling phase)
β€’ 3–4 sec slow lower
β€’ 3 sets of 12–15
β€’ 1x/day
β€’ Mild discomfort is okay if it settles within 24 hrs

3️⃣ Heavy Slow Isotonic Wrist Extension (Capacity phase β€” most important)
β€’ 3 sec up / 3 sec down
β€’ 3–4 sets of 8–12
β€’ 3–4x/week
β€’ Progress load weekly

Save this if gripping, lifting, or pickleball flares your outer elbow.
Load the tendon progressively > random stretches + rest.

It moves 1–4 degrees.

When it gets irritated, it’s usually a muscle imbalance + load issue.

Muscle Energy Technique (MET) helps by:
β€’ Gently activating specific muscles
β€’ Reducing guarding
β€’ Improving pelvic symmetry
β€’ Decreasing shear forces through the SI joint

We’re not β€œputting it back in.”
We’re restoring better force transfer.

πŸ’ͺ The Brandt-Daroff exercise is a simple, evidence-based habituation movement that helps your brain and inner ear adapt to those positional changes. Over time, it can reduce dizziness episodes and improve tolerance to movement.

Slow, controlled reps.
Let symptoms settle between positions.
