Aging is inevitable (and a privilege!). Looking like it happened overnight doesn’t have to be.

I’m turning 42 this week, and my approach is simple: start early with a dermatologist that you trust. Make a plan and reassess your needs at regular intervals.

The goal? Look younger, age slower and look like the best version of yourself.

Scared of the minoxidil dread shed? šŸ‘€ Don’t let it cost you your hair.

Minoxidil is one of my go-to treatments for hair loss—and I personally use it every single day.

Yes, that initial shed can be scary. But it’s been proven that patients who experience more shedding early on often end up being some of the best responders.

So as with everything, patient patients win. But as someone who is not very patient, I get it. šŸ˜‚

If the shed is still holding you back, here’s the move: start minoxidil alongside in-office growth factor treatments like PRP or Kerafactor.

They can help minimize that initial shed, stimulate growth on their own, and potentially give us some serious synergy.

Think: 1 + 1 = 5. šŸ“ˆ

In my practice, the vast majority of patients starting this combination are already seeing positive progress around the 8-week mark. So no need to wait six months for those results!

Don’t let a temporary shed scare you away from a treatment that could help you keep—and grow—more hair long term.

Your hair is shedding like CRAZY right now? There’s a good chance summer is to blame. šŸ‚

Human hair actually has a seasonal rhythm. As we enter summer, more follicles get the signal to enter their resting phase. But just like you with your morning alarm, they hit snooze a few times before finally packing up.

2–3 months later? Mass exodus. šŸ‘‹ And that means shedding tends to hit a fever pitch from August through October.

The good news: seasonal shedding is normal, temporary, and those hairs should grow back.

But here’s the catch: this has been happening your entire life.

If you’re suddenly noticing it much more than you used to, you may have already lost some underlying density—often from genetic hair loss—making a normal seasonal shed much more obvious.

If that sounds like you, or your shedding isn’t letting up, see your board-certified dermatologist. Earlier diagnosis = more hair to protect.

Is creatine making you bald?Ā šŸ’Ŗāž”ļøšŸ‘Øā€šŸ¦²

This question must be circulating the Reddit boards because I’ve been getting it A LOT lately.

First things first: creatine isn’t going toĀ causeĀ genetic hair loss because, well… genetics.

Genetics essentially flips the switch in those who are susceptible, making their hair follicles sensitive to DHT—even at completely normal DHT levels.

So where does the creatine concern come from?
A small study published in 2009 found an increase in DHT after creatine supplementation. And since DHT drives follicle miniaturization in genetic hair loss, the concern became: could creatine accelerate hair loss in someone who is already susceptible?

The problem?

The study included only 20 guys and lastedĀ just three weeks.

Even more importantly, the creatine group started with substantially lower DHT levels. Their DHT then ā€œrose,ā€ butĀ remained within normal physiologic levels.

That makes it pretty tough to take a short-term change and conclude that creatine is meaningfully increasing the DHT exposure that drives genetic hair loss.

Fast-forward to 2025, and we finally got a much better randomized, placebo-controlled trial.

Men took a standard 5 grams of creatine daily for 12 weeks.
The result?

No significant increase in DHT. No significant difference in testosterone. And no significant difference in hair density, thickness, or other measured hair outcomes versus placebo.

So, could creatine be accelerating your genetic hair loss?
Based on the best evidence we have right now:Ā probably not.

If you have genetic hair loss, you need an effective plan to protect your follicles from DHT. Otherwise, you’re cooked. 🫠

ā€œDoc, will finasteride kill my gains?ā€Ā šŸ’Ŗ

A question I hear from my gym-going hair loss patients ALL the time.

The short answer:Ā the evidence says no.

Finasteride inhibits 5-alpha-reductase, decreasing the conversion of testosterone to DHT. While DHT drops, testosterone stays the same or may actually increase slightly.

And here’s the important part: skeletal muscle has very little 5-alpha-reductase activity. Testosterone can act directly on androgen receptors in muscle without needing to be converted to DHT.

More importantly, we have randomized human data to back that physiology up. Even when DHT production was profoundly suppressed, researchers found no meaningful difference in gains in lean muscle mass or strength. Studies combining testosterone with finasteride have similarly shown preserved gains in muscle and strength.

DHT matters a lot in certain tissues — including the prostate, skin and hair follicle. Skeletal muscle? Not so much.

Bottom line: finasteride lowers DHT. It doesn’t lower your gains.Ā šŸ™Œ

And yes, judging by those 3 lb dumbbells, I recognize that I may not be the ideal spokesperson for gains. šŸ˜‚

Even the best sunscreen routine isn’t completely sunburn-proof. ā˜€ļøšŸ”„

Take it from your pasty Irish dermatologist who may or may not—but definitely has—dealt with his fair share of sunburns 😬

Prevention is still your best defense. Use a broad-spectrum SPF 30 or higher and reapply every 2 hours. Add sun-protective clothing whenever you can. And for those who burn easily, consider a Polypodium leucotomos supplement as an internal assist to help boost your skin’s defenses against UV damage—not as a replacement for sunscreen.

But if the sun still catches up with you… what actually helps?

My friend and beauty guru @samholender put together a great guide for @marieclairemag on exactly what to do when you get burned, and was kind enough to tap myself and all āļø colleague @drmarisagarshick for our dermatologist-approved tips to help your skin recover.

Yes, she got filler. No, you shouldn’t be able to tell. šŸ‘€

The goal isn’t to change your face. It’s to restore what time has taken away.

Volume loss is a fundamental part of facial aging, and filler or fat transfer are the only tools we have to actually replace that lost volume.

Filler has gotten a bad reputation lately. And frankly, the industry bears some responsibility. In the race to sell more product, filler companies ā€œtrainedā€ a generation of injectors — and somewhere along the way, more filler became the goal instead of better outcomes.

Add in perception drift, and you get a lot of overfilled faces.

Sometimes the most important thing an injector can do is say no. I say no a lot.

Because with filler, who holds the syringe matters.

If your injector looks like an alien — or their before-and-afters do — run. šŸ˜‚

Good filler should respect your anatomy, restore support where it’s been lost, and enhance your natural features — not create new ones.

Strategic placement. Small amounts. No need for 100% correction.

Her review? ā€œI look like I actually had sleep!ā€

That’s good filler.

And the good news? If you’ve fallen prey to bad hyaluronic acid filler, in many cases we can dissolve it and start over.

Do your homework. Choose your injector carefully. But don’t be afraid of a great treatment because you’ve seen a lot of bad filler.

The best filler is the filler no one knows you have.

Oral minoxidil has earned the hype. šŸ‘€

Thanks to @joleneewrites and @allure for including me in this deep dive on one of my go-to treatments for hair loss.

And when we’re looking to really maximize results? Oral minoxidil + ScalpStim Pro is one of my favorite combinations in practice.

Keep swiping — slide 4 is why I’m such a believer in combination therapy. šŸ‘€

The wedding was in June… but my video editor (who also happens to be the bride) apparently decided to take a post-wedding sabbatical. šŸ˜…

So here she is—the final product. One absolutely beautiful bride. ā¤ļø

In the months leading up to the big day, our goal was never to make her look different. It was to help her look like the best version of herself.

Our plan included:
✨ Dissolving overdone filler, then restoring subtle, undetectable volume to enhance her natural features.
✨ Laser resurfacing for brighter, smoother skin.
✨ Neuromodulator to soften lines, reduce hairline sweating (because wedding makeup deserves a fighting chance), and refine the jawline and neck.
✨ PRP to support stronger, healthier hair.

The biggest takeaway? The best cosmetic results rarely come from a single treatment. They come from a thoughtful, personalized plan that enhances your natural features—not changes them.

Congratulations again to my sister and her new husband. Now that the wedding content is finally wrapped up… can I have my video editor back? šŸ˜‚

Part 2: My favorite ACTIVE sunscreens ā˜€ļøšŸ–ļø

Your daily sunscreen probably isn’t designed to be water resistant—and that’s completely okay.

But if you’re heading to the beach, playing sports, spending the day outside... or trying to survive 18 holes in the middle of summer ā›³ļøā˜€ļø, it’s time to switch to a sunscreen with 80 minutes of water and sweat resistance.

These are the two I keep reaching for:

🟢 EltaMD UV Sheer SPF 50+ – Lightweight, beard approved, and one of the best water-resistant formulas I’ve used. They throw in antioxidant protection with vitamin C as the šŸ’ on top.

šŸ–ļø ISDIN Eryfotona Actinica SPF 50+ – A great option if you don’t mind a slight white cast. Pure mineral base with added DNA 🧬 repair enzymes. This is a must if you have a history of skin cancer, precancerous spots, or years of sun exposure.

One lives in my beach bag, golf bag, and tennis bag for a reason.

What sunscreen should I review next?
