Since @anesthesiagawd asked, here’s a demonstration of why rotating a nasal endotracheal tube can help your placement be smoother, easier, and far less traumatic.

This is a great technique and must-have skill, especially for office-based CRNAs who need to ensure quick and gentle airway placement.
You will also ensure cuff integrity because no Magill forceps are used! Give it a try!

I attended my first MYA in 2017 and was HOOKED.

I experienced what a group of passionate CRNAs could achieve together and I knew that I wanted to be a part of that.

I have gone nearly every year since and am in my second year as a Federal Political Director for my state.

MYA taught me the importance of advocacy and legislation in our daily practice.
✅ Do you like your paycheck? 💰That’s advovacy
✅ Do you like your autonomy? 🧑‍⚕️ That’s advocacy.
✅ Do you want a bright future for CRNAs? 📚 That’s advocacy.

Advocacy is also way easier than you think. It’s just having a conversation educating people about what we do every day. You’re the experts in Nurse Anesthesiology - why don’t YOU make sure lawmakers know who YOU are?

I’ll be speaking on Saturday AM but the whole week is full of educational content for beginners and experienced CRNAs alike. CEUs are offered for almost everything - except the PAC party (which shouldn’t be missed!) 💃🕺

I’m excited about going and passionate about how our voices can shape the future of Nurse Anesthesiology.

I hope to see you in Washington, DC April 26-30!!!

Tired of weak bite blocks that don’t work?

For my first reel (!!!) let me show you my method for ensuring that your patients stay safe and don’t occlude their endotracheal tube.

I call this one the SandmanCRNA method 😜

Try this out and let me know how it works for you!

1099 vs. W2: Which is Right for You?💰💼

As a nurse anesthesiologist, your employment type affects your income, taxes, benefits, and flexibility. Are you looking for stability with employer benefits (W2) or higher earning potential and more control over your schedule (1099)?

I work full-time 1099 and I love it, though there is a considerable amount of work owning a business. The payoff for me is flexibility, pay, and client selection.

Swipe through to see the key differences and what might work best for your career goals! 📊

💡 Pro Tip: If you’re considering switching, make sure you understand tax responsibilities and retirement options before making the leap. DM me to discuss!

✨ Which one do you prefer—1099 or W2? Comment below! ⬇️

INTRAOPERATIVE HYPOTHERMIA.

It’s a lot more than just “keeping people comfortable.”
I prioritize normothermia in EVERY anesthetic that I deliver and here is why:

Hypothermia leads to a variety of problems. Patients are not only cold, shivering, and uncomfortable, they’re at risk for cardiac events, coagulopathy, delayed emergence, and much more.

Henry’s Gas Law dictates that as the temperature DEcreases, solubility INcreases inside a liquid. Translate this to an inhaled anesthetic gas and plasma, and hypothermic patients will have longer times under anesthesia because the gas simply doesn’t want to leave the bloodstream. We know of an association of age, time under anesthesia, and increased postop cognitive dysfunction (POCD). In theory, we can mitigate this risk by keeping patients normothermic and reducing anesthesia emergence & recovery times.

Surgical stress doesn’t stop with the emergence of anesthesia. A shivering patient is exponentially increasing their myocardial oxygen demand and is at a greater risk of adverse cardiac events - even after the anesthetic is off.

New literature suggests that a core temperature of 35.5°C may have no different outcomes - I’m curious and reluctant to adopt new practice based on one study (even if it was a good one!). Make your own judgements here.

Preoperative forced-air warming has been shown to reduce postoperative hypothermia and you’ll notice that many ERAS protocols include preop warming in the stepwise approach to improve outcomes.

Beyond patient comfort, hypothermia has real consequences that we must be sure to avoid. These are just a few of the many reasons we should be focusing on ensuring every patient is normothermic throughout the operation and upon arrival to the PACU.

The age-old question: Does size matter? Or is is just how you use it?

Poiseuille's Law describes fluid flow through a cylindrical pipe. It helps us understand how different factors affect flow rate.

When applying this to anesthesia, we tend to think about the law in terms of IV sizes. Most IVs range in size from 24G (small - think: neonates) to 14G (very large - can even transduce a 14G in the basilic vein as a proxy for CVP!)

Larger catheters allow for increased flow rate because of their radius. The r^4 means that small increases in size lead to BIG changes in flow.

Other factors like length, pressure, and fluid viscosity are also important.

Think about a 16G on a triple-lumen CVC. It should flow fast because it’s a 16G, right? Nope. It flows about as fast as a 20G PIV. Length is a factor here.

When running fluid in a trauma situation, we use pressure bags or rapid transfusion systems to get fluid in quickly. Sometimes just elevating the IV pole height helps increase the ΔP to achieve faster flow rates.

Packed Red Blood Cells are thick. We can dilute them with crystalloid (NS or Plasmalyte) to increase flow by making the fluid less viscous. Otherwise it’s like trying to drip cold molasses into the IV.

Consider the surgery, anticipated blood loss, patient condition, and fluid administration rates when choosing what size IV to place. Remember: Optimal catheter selection balances patient comfort and clinical needs!

ESMOLOL: it’s a Beta-blocker, traditionally used for cardiac conditions, but it offers exciting possibilities in postoperative analgesia.

Numerous studies have explored the use of ESMOLOL as an adjunctive therapy for postoperative analgesia. It’s use aims to reduce opioid use, reduce pain scores, and improve outcomes.

We don’t have all the answers yet, but there is a growing bed of evidence that suggests promise for the use of IV ESMOLOL for postoperative analgesia. It’s inclusion on many ERAS protocols suggest that it can improve recovery and perhaps even hospital length of stay.
