All-On-X Approaching Final Stages!

In a fixed full-arch case, implant placement is only part of the equation. Two principles I pay close attention to are soft-tissue health and A-P spread (anteroposterior spread).

Healthy, stable peri-implant soft tissue creates the biologic foundation around the prosthetic connections. We want tissue that heals predictably, remains maintainable for the patient, and provides adequate access for long-term hygiene. The goal isn't simply osseointegration—it's creating an implant-prosthetic environment that can remain healthy and cleansable over time.

Then comes the biomechanics.

A-P spread describes the distance between the most anterior and most posterior implants supporting the prosthesis. Maximizing that spread within the patient's anatomy helps distribute forces across the arch and can reduce the cantilever required behind the terminal implants. Implant number alone doesn't tell the whole story—where those implants are positioned matters.

That's why full-arch treatment is more than simply "placing implants."

It's a combination of:

Biology + biomechanics + prosthetic planning.

When all three are planned together, we're not just trying to create a beautiful smile—we're designing a foundation for function, hygiene, tissue health, and long-term stability.

HERE’S A 3 WEEK FOLLOW UP!

Case review: extraction + immediate implant placement, site #19.

Non-restorable mandibular first molar → atraumatic extraction → immediate implant placement into the fresh socket, same day.

At 3 weeks, soft tissue healing shows a dense, stippled band of keratinized mucosa forming a tight cuff around the healing abutment — no inflammation, no recession, clean margins.

Keratinized tissue width around an implant isn’t cosmetic detail — it’s one of the stronger predictors of long-term peri-implant health, giving better resistance to plaque-induced inflammation and mechanical irritation than thin, mobile alveolar mucosa.

This is the tissue quality we’re aiming for at every immediate placement.

A wise man once said “Tissue is the issue”—you gotta protect it!

Honored to present at PNDC on the role of nutrition optimization in oral surgery and how targeted nutritional strategies can enhance healing, reduce complications, and improve patient outcomes.

As surgeons, our work doesn’t end when the procedure is complete. Proper nutrition before and after surgery plays a critical role in tissue repair, immune function, and overall recovery.
