AGGRESSIVE MULTIMODAL SAME DAY ACNE SCAR TREATMENT @shanthalamd @dr_shanthala
ARE YOU READY FOR THE DOWNTIME?

Here is one example:

1️⃣ Skin of color + significant acne scarring
2️⃣ 27 years old — first acne scar treatment
3️⃣ 4–5 advanced modalities combined in one session
4️⃣ Before → Immediately After → Day 3 → Day 5
5️⃣ Day 5 = healing in progress, NOT the final result

In skin of color, careful pretreatment planning is critical.

Key concerns:

1️⃣ Post-inflammatory hyperpigmentation (PIH)
2️⃣ Prolonged inflammation
3️⃣ Delayed healing
4️⃣ Infection
5️⃣ Rarely, abnormal scarring

This is why consultation and careful patient selection matter.

Every patient heals differently. If you are considering an aggressive treatment, you must be prepared for the downtime and committed to following the post-care protocol.

Significant acne scarring may require 3–6 combination treatments for visible improvement. taken in Boston, Massachusetts by @shanthalamd

1. Can you predict audible resistance before subcision?
Not really. You can see the scar on the surface, but you do not truly know the underlying mechanical resistance until you enter the tissue plane.
2. Does audible resistance predict success?
Not by itself. Hearing resistance means you are encountering fibrotic tissue, but louder or more aggressive subcision does not automatically mean a better result.
3. If there is no audible resistance, does that mean less scarring?
No. A severely atrophic scar may have very little resistance if the dominant problem is tissue loss rather than dense tethering. taken in Shanthalamd Laser & Aesthetic Center by @shanthalamd

“I chose Dr. Shanthala—and I would do it again in a heartbeat.”

Mary didn’t jump into laser skin resurfacing lightly. She explored all her options—technologies, treatments, and providers. But what made her choose me was more than just the results.

She felt seen, heard, and cared for. She saw the precision. The honesty. The attention to detail. The commitment to safety and real, natural-looking outcomes.

And she chose Ultraclear Laser Coring because she didn’t want a quick fix—she wanted a transformation. A procedure that would truly renew her skin without surgery, with fast recovery and long-term benefits.

Four days after treatment, she was already back at work—glowing. And she said it best: “I just can’t say enough about the results… and I can’t say enough about her as a provider and as a person.”

Mary, thank you for trusting me. It means everything.
If you’re considering non-surgical skin rejuvenation, and you want a provider who truly cares—I’m here for you.

Vertical upper lip lines are not all treated the same way

✅ Some lines are mainly caused by muscle movement
That may be where a neuromodulator has a role.

✅ Some lines are associated with volume loss or support loss
In selected patients, filler may help.

✅ Some are deeply etched into the skin
These often need resurfacing rather than simply adding volume.

✅ Some patients have significant laxity or deeper structural change
That is where more advanced treatments, including laser enabled tissue coring or deeper resurfacing, may be considered.

✅ Full ablation is not a treatment chosen casually
There are published protocols, expert consensus recommendations, clinical endpoints, and safety principles that guide how these treatments are performed.

✅ There is no single recipe
Different lasers behave differently, different skin types heal differently, and different physicians may use different protocols based on the patient in front of them.

❌ It is not simply:
“See a wrinkle → choose a laser → choose a depth.”

The decision includes skin quality, depth of the lines, muscle activity, volume loss, skin type, healing ability, previous treatments, expected downtime, and the physician’s examination.

And this photograph is simply Day 5 of healing. It is not the final result, and another patient may heal faster or slower.

If these vertical lip lines bother you, come in for a consultation. The first question is not “Which procedure?” The first question is “What is actually causing your lines?” taken in Boston, Massachusetts by @shanthalamd

76 years old and gorgeous, Day 3 after LaserCoring.

What makes this testimonial especially meaningful to me is that she is not new to aesthetic treatments.

Over the years, she has experienced many different procedures — including Fraxel, microneedling, Ultherapy, thread lifts, multiple types of fillers and biostimulators such as Sculptra and Radiesse, and even CO2 laser resurfacing.

So when she talks about this experience, she has a great deal to compare it with.

And here she is, just 3 days after LaserCoring, sitting comfortably and talking as though very little happened. She even tells us she went out to lunch on Day 2.

I want to be very clear: this degree of recovery is unusual, and I cannot predict or promise that every patient will heal this way. Every patient, every treatment, and every recovery is different.

I am simply proud to share her experience, and incredibly grateful that she was willing to speak so openly and share her personal perspective.

She has never had facial surgery. She had considered a facelift, but what matters most to her is looking refreshed without looking pulled, altered, or unlike herself.

This is her story.
Her experience.
Her opinion.

And I am very grateful she allowed me to share it.

A visible refinement in under two weeks.

This is not the correction of a single concern. What you are seeing is a comprehensive improvement in skin quality—texture, laxity, fine lines, crepiness, and photodamage—across the face and neck, including the most delicate areas around the eyes and mouth. These changes reflect extrinsic aging—the cumulative impact of sun exposure and environment—often far more significant than intrinsic aging alone.

The Boston Laser-Coring Lift™ is a customized, physician-performed protocol designed to address the skin at multiple depths with precision and restraint. Not every patient requires every device. The value lies in understanding what is appropriate, what is not, and how to approach the skin as a whole rather than in isolated parts.

In carefully selected patients, even a single treatment can lead to a meaningful shift:
• Smoother texture
• Improved firmness
• Softening of fine lines
• A more even, refined appearance

All consultations, procedures, and follow-ups are performed exclusively by Dr. Shanthala Shivananjappa, MD, ensuring continuity, safety, and a highly individualized experience throughout.

This is not about excess.
This is about treating the skin correctly—and maintaining it with intention over time.

Severe atrophic scars are architectural problems, not just surface problems.

Case: Severe atrophic acne scarring with significant volume depletion.
Challenge: Atrophic scars are not just textural. Fibrous bands tether the dermis to deeper structures, creating fixed depressions. Volume loss further accentuates shadowing and contour irregularity. Single-modality treatments rarely address both components.

Approach by Dr. Shanthala, MD: Several sessions of same-day multimodal therapy combining Subcision, PRP, Fillers, and LaserCoring.
✅Subcision: Physically releases subdermal fibrotic adhesions to elevate the scar base and create space for collagen deposition.

✅Volume Restoration: Hyaluronic acid filler placed post-subcision occupies the subcision pocket, preventing re-adhesion while restoring lost volume. PRP provides growth factors to support tissue regeneration.

✅LaserCoring: Fractional ablative technology excises full-thickness columns of scarred skin. The controlled removal and healing recruits normal collagen and elastin, improving texture and dermal architecture over time.

❓Why same-day? Combining release, volume, and resurfacing in one visit addresses the scar at multiple structural levels while leveraging the wound-healing cascade once.

💯Important: How Dr. Shanthala approaches each case and which order varies by patient anatomy and scar type. Come in for consultation to determine the right plan.

Why laser coring — and not CO₂? Why did Dr. Shanthala recommend it?

This 74-year-old patient had never done anything cosmetically before.

Two years ago, she came to me looking for meaningful facial rejuvenation. After evaluating her skin, laxity, face and neck, I recommended UltraClear laser resurfacing with laser coring.

Now, at her 3-month follow-up, she is sharing her own experience — and she is absolutely thrilled.

Her improvement has been one of the most robust I have seen: remarkable full-face rejuvenation, significant improvement in the neck and lower face, and beautiful skin quality — without injectables.

This is exactly why I believe laser coring is changing the conversation around advanced laser rejuvenation.

The future is not simply about using more energy. It is about using better technology, better tissue targeting, and better patient selection.

Very grateful to this gracious patient for allowing me to share her story.

Advanced Acne Scar Remodeling
During almost every acne scar consultation, patients tell me the same thing:

“I have realistic expectations. I don’t expect perfect skin. I just want to see visible improvement.”

Then come the questions…

Hundreds of them.

Every question is valid, and every patient is different.

We’ll talk about all of them during your consultation ✅

One Year Later: Even Better Than Before!

Today was such a special moment—my patient returned for her 12-month follow-up after her @ultraclearlaser (Laser-Coring) treatment, and the results are nothing short of incredible.

In fact, her skin looks even better today than it did at 3 months post-treatment. So much so, that people have asked, “Are you sure this is the same person?” And I smile, because yes—it absolutely is. The transformation is real, and it’s naturally beautiful.

Her results are a testament to the power of UltraClear Laser, and the incredible things your skin can do when given the right tools and time to heal.

Today she came in for her second maintenance session—a gentle follow-up after a year—and I can’t wait to see how she continues to glow. I’ll be sharing her video soon, so keep an eye out.

If you’ve been thinking about reclaiming your skin without surgery, don’t wait. The longer we delay, the harder it gets to reverse the clock. Let this be your year.

I’m not here to boast—I’m here to invite you to see for yourself:
	•	Look at my before-and-afters.
	•	Read what my patients say.
	•	Ask questions.

I promise you this: you’ll walk out looking like you—only better.

If you’re ready to truly feel good about your skin in 2025, UltraClear Laser may be the best decision you’ll make for yourself.
