It is Medal Monday again, and this time is quite a special one! I have to show not 1 but 2 new Medals (or better say 7 overall? 🤔). 
Yesterday I finally have completed the @super_halfs ! 
The journey hasn't been the smoothest. It started with Valencia 2021, which wasn't considered valid because not all the other races on the circuit were run that year due to the pandemic, so I had to run it twice. 
Then initially they had to be run within a certain timeframe so all of them always fell in less than ideal times, sandwiched between my other commitments and the Majors project, finally they became lottery only. 
The result is that I never ran them or planned them as goal races.  The same happened here, three weeks after Tokyo, two weeks from Milan, and less than two months from Cape Town. 
My goal was to complete the circuit, and we did it! Many asked me: were you the first diabetic? Or the fastest? 
Maybe... I honestly don't know. Unlike the Majors, it's not easy to verify this time, and I am sure there are dozens of T1Ds with faster halfs times than me and for sure more of them running Halfs rather than Full Marathons, so I wouldn't bet on it. 
For me these moments are not always about Guinness or records, but more about showing what we can do despite our condition, inspiring and giving hope to those living with it and raising awareness for those who don't know it or the obstacles it poses. Meanwhile another wall has fallen!
And more than medals for me the most valuable takeaways are memories of the moments people and friends share with me. 
Some a whole day, others a meal, a run, or part of the race. 
Some are old friends and others new ones, but all of you contributed to make this weekend special. Unfortunately I do not have pictures with all of you but you know who you are and I have to say thank you, from the bottom of my heart 🙏🏻💙

On November 5th, 2023, I completed the 6 World Marathon Majors in New York with an overall aggregate time of 16:14:59 (average 2:42:30 per marathon), setting a Guinness World Record as the fastest runner ever with Type 1 Diabetes to complete the 6 most important Marathons in the world.
I had the honor of having my story featured by @wmmajors in a video made for World Diabetes Day.
It was an experience and a memory that I will carry with me forever.
I hope that my story will inspire many other diabetic athletes not to give up on their dreams, but also non-diabetic people to get involve and never give up. 
I think I have proved that with the right training, nutrition, therapy, strategy and the help of the latest technologies such as @abbottglobal @freestylediabetes Freestyle Libre 3 we can achieve results that were not even imaginable just a few years ago.
This was made possible through research, and that's why I supported and will continue to support @jdrfhq

I can finally say I am now a Six Star Finisher! This journey started during a training run on London's Mall (the finish line of the London Marathon) at the end of November 2019, few days after my first sub 3 in Florence. 
It took me 1435 days, 205 weeks or just over 47 months of persistent training in every kind of conditions (including a pandemic).
I have experienced any kind of adversity in my training and races (torrential rain, snow, extreme cold, extreme hot, very strong wind, injuries, running an entire marathon needing to pee, running an entire marathon needing to poo, being knocked down and trampled to the floor in a target race). 
Plus I had of course all the additional challenges due to Diabetes (very poor sleep, slow recovery from hard training and injuries, dehydration, needing to drink and pee often, ketoacidosis, hypoglicemia, hyperglicemia, etc). But I never gave up, no matter the challenges.
This moment was so special to me that I wanted my father to be there, so he was waiting for me at the finish line, and of course I got emotional when I got my 6 star medal around my neck as you can see from the pictures! 
During this journey I was lucky enough to meet amazing people and being exposed to so many inspiring stories that changed my life in many ways.
I cannot mention them all, you know who you are! Special thanks to my dad  @sergio.carniglia that always supported me, to my friend and physio @lolo_bene91 that helped me staying consistent during my training and to my coach Bea. 
This is the end of  journey and the beginning of a new one... I have already set my next challenge... Stay tuned 😉

Welcome to Ep. 3 of "The Unseen Variables".
 Did building more muscle cause my blood sugar to crash?
​For the first time in 27 years of living with T1D and 8 years of running, a strategy specifically adapted for the heat (which seemed to be working) completely failed me. 
Last Saturday, I headed out for a long run. 
I did everything by the book to prevent a drop: I significantly reduced my basal insulin, I had ZERO active rapid insulin in my system (for over 24 hours!), and I fueled with plenty of sugar.
​It wasn't enough. Halfway through, I crashed hard. I had to cut the run short, eat even more carbs, and literally sit on the sidelines waiting for my numbers to go up just to make it back home safely.
​The summer heat is definitely still playing a huge role here, but it can't be the only variable anymore.
​My hypothesis? Body composition. 
Since Tokyo, I’ve lost about 5 kg. 
I’ve been hitting the gym a lot more, focusing heavily on strength training, and I am visibly leaner with more muscle mass. 
Muscle tissue is metabolically expensive, and my theory is that at the exact same pace, my "new engine" is just chewing through sugar way faster.

​Someone in the community suggested I should try splitting my basal insulin dose in two to fix this.

🇮🇹Tempo di annunci! La prima maratona a cui mi sono iscritto per il 2027 è la @milanomarathon !
Perché Milano? Perché sarà un evento speciale per la 25° edizione della gara e perché ci sono molto legato. 
È stata la mia prima maratona nel 2019, la mia 20° nel 2025, e ho partecipato anche a diverse altre edizioni con staffette o usandola come lungo in preparazione di altre gare oppure facendo il tifo!
Quindi non potevo fare a meno che correre l'edizione dell'anniversario!
Se ancora non hai idea di cosa fare, unisciti a me e corri a Milano!
Ci aspetto sulla linea di partenza il 4 Aprile 2027!

🇬🇧Announcement time! The first marathon I've signed up for in 2027 is the @milanomarathon !
Why Milan? Because it will be a special event for the 25th edition of the race, and because I'm very attached to it.
It was my first marathon in 2019, my 20th in 2025, and I've also participated in several other editions, either as a relay team, or as a long run in preparation for other races, or as a supporter!
So I couldn't help but run the anniversary edition!
If you still don't know what to do, join me and run in Milan!

🇮🇹 Ma come sono finito in uno sprint al fotofinish con @giomaionee ? 
L'ho fatto al Velodromo Vigorelli partecipando alla tappa Milanese degli ASICS Blast Champs per la presentazione della nuova #NOVABLAST 6
Le abbiamo potute testare sfidandoci con 6 squadre di 6 corridori impegnate in una staffetta di 800m, ma con una variante particolare. Ogni atleta alla fine dei due giri di pista doveva rispondere ad una domanda sulla corsa, e in caso di risposta errata scattava la penalità ed il testimone poteva essere passato al compagno solo dopo il tratto extra di corsa.
Gli ultimi frazionisti, come noi, dovevano solamente correre verso il traguardo, ed è andata a finire così! Penso che ce lo ricorderemo per sempre! 😝

Dopo la corsa tanti sorrisi, abbracci  la musica di @djaladyn.
Le #NOVABLAST 6, grazie alla nuova combinazione di mescole FF TURBO SQUARED e FF BLAST MAX si è rivelata più reattiva e stabile del modello precedente, ma anche molto leggera grazie alla nuova tomaia in tessuto ingegnerizzato, perfetta per una corsa come questa!
Sono certo che faremo molti Km insieme verso i miei prossimi traguardi 👀

🇬🇧 How did I end up in a photo-finish sprint with @giomaionee?
I did it at the Vigorelli Velodrome while participating in the Milan stage of the ASICS Blast Champs for the presentation of the new #NOVABLAST 6.
We were able to test them by competing against six teams of six runners in an 800m relay, but with a special twist. Each athlete had to answer a question about the race at the end of the two laps, and if they answered incorrectly, they were penalized, and the baton could only be passed to their teammate after the extra stretch.
The last runners, like us, just had to run to the finish line, and that's how it went! I think we'll remember it forever! 😝

After the race, lots of smiles, hugs to the music of @djaladyn.
The #NOVABLAST 6, thanks to the new combination of FF TURBO SQUARED and FF BLAST MAX compounds, has proven more responsive and stable than the previous model, but also very light thanks to the new engineered fabric upper, perfect for a run like this!

Welcome to Ep. 2 of "The Unseen Variables". 
Two runs, two crashes, one hidden culprit.
​Yesterday, I crashed at km 14 and had to call a car for a rescue. I started at 300 with zero active insulin, but didn't take gels.
​So today, I tried the "perfect plan" to fix it: 8 AM early start, blood sugar at 250, zero active insulin, AND I took a gel right at the starting line.
​The result? I crashed in the exact same spot.
​When you keep crashing even after fueling up, eating more gels isn't the answer—it just destroys your stomach. 
The real issue is the background insulin. 
The physical effort combined with the summer temps makes the basal insulin way too aggressive.
​Next time, the strategy changes: I’m dropping the basal insulin and keeping the fuel.

🇬🇧Friendship can be the answer to many questions. For me one is the following: Why travelling over 400km and 5hr drive (each way) to run an apparently random 8,5Km race at 19:30 with 40°C and 90% humidity on the hottest weekend of the year? 
Because it was not a random race, but the @corsa_pescatore_scardovari organized by @guardianodelparadiso and @podisti_porto_tolle that became friend of mine many years ago in Valencia. I always promised to join his race but for different reasons I never managed to.
This year I had the chance and so I kept my promise, and I am glad I did!
Not only I got to meet Angelo and @andrea.zeta.86 , but also @giorgia_mancin and many more.
Plus It was the chance to visit a place I have never been and explore a bit, stay a few hours on the beach and to share some days with my father @sergio.carniglia 
I highly reccomend to join this race,  incredibly well organized and nice.
And I mean: where else you see such a large prize bag for the 11th Age Group Place? 😛

🇮🇹L'amicizia può essere la risposta a molte domande. Per me una di queste è: perché percorrere oltre 400 km e guidare per 5 ore (solo andata) per partecipare a una gara apparentemente casuale di 8,5 km alle 19:30 con 40°C e il 90% di umidità nel weekend più caldo dell'anno?

Perché non era una gara casuale, ma la @corsa_pescatore_scardovari organizzata da @guardianodelparadiso e @podisti_porto_tolle, che sono diventati miei amici molti anni fa a Valencia. Avevo sempre promesso di partecipare alla sua gara, ma per diversi motivi non ci ero mai riuscito.

Quest'anno ho avuto l'opportunità e ho mantenuto la promessa, e sono contento di averlo fatto!

Non solo ho incontrato di nuovo Angelo e  @andrea.zeta.86, ma anche @giorgia_mancin e molti altri.

Inoltre, è stata l'occasione di visitare un posto in cui non ero mai stato ed esplorarlo un po', passare qualche ora in spiaggia e trascorrere alcuni giorni con mio padre @sergio.carniglia.
Consiglio vivamente di partecipare a questa gara, incredibilmente ben organizzata e bella.

E poi: dove altro trovate un pacco premio così ricco per l'undicesimo posto di categoria? 😛

Managing the heat isn't just about hydration; it’s about adjusting the timing and the dosage before I even tie my shoes.

​⚠️ Side note: Starting with higher blood sugar to compensate is risky if you don't check for ketones first.

🇬🇧Today @londonmarathon announced that the 2027, for one year only, will be held on two consecutive days: Saturday April 24 and Sunday April 25!
If you applied for the ballot earlier this year you will be entered in two different ballots automatically: one for each day.
You cannot run them both, the allocation will be random.
For the Elite Fields Women will run on Saturday while Men will run on Sunday.
This has been done to increase the chances to get in with the ballot since the number of applications was over 1,3 Millions.
The chance for non UK residents is still under 2% but I guess that for UK resident that opted for the double chance for the ballot the oddio are quite good is basically like having 4 entries!

🇮🇹Oggi la maratona di Londra ha annunciato che l'edizione del 2027, solo per un anno, si terrà in due giorni consecutivi: sabato 24 aprile e domenica 25 aprile!

Se avete partecipato al sorteggio all'inizio di quest'anno, sarete automaticamente inseriti in due sorteggi diversi: uno per ogni giorno.

Non è possibile correre entrambi i giorni, l'assegnazione sarà casuale.

Per gli atleti Elite, le donne correranno sabato mentre gli uomini correranno domenica.
Questa decisione è stata presa per aumentare le possibilità di essere inseriti nel sorteggio, dato che il numero di candidature ha superato 1,3 milioni.

Le possibilità per i non residenti nel Regno Unito sono ancora inferiori al 2%, ma immagino che per i residenti nel Regno Unito che hanno optato per la doppia possibilità di sorteggio le probabilità siano piuttosto buone, praticamente come avere 4 biglietti della lotteria!

🇬🇧 They look like "just 3 medals", but there's more behind them. 
8 Marathons across 5 Continents, the @wmmajors, the most important, the most desired and sought for in the world. 
Just taking part in one of them is a dream for many. 
Right now in the world only 1994 athletes (including @kipchogeeliud ) own these 3 medals and are officially 8 star finishers.
101 of them are from Italy, only 1 in the world has Type 1 Diabetes, and that is me.
This is a message for those that, like myself, live with some health challenge: don't give up on your dreams!
It is harder, we face more variables and risks, but we can still achieve incredible things!

My journey is not over yet, I will keep running and working towards inclusivity for T1Ds 💙

PS: I usually don't share this kind of data, but I cheched the fastest aggregate times for the 101 italians and the 1994 in the world. I should be 1st Italian and in the top 15 in the world. I verified the Italians, while the international was done by AI 👀

🇮🇹Sembrano "solo 3 medaglie", ma dietro c'è molto di più.

8 maratone in 5 continenti, le @wmmajors, le più importanti, le più ambite e ricercate al mondo.
Anche solo partecipare a una di queste è un sogno per molti.
Attualmente, al mondo, solo 1994 atleti (tra cui Eliud Kipchoge) possiedono queste 3 medaglie e sono ufficialmente 8 star finishers.
101 di loro sono italiani, solo 1 al mondo ha il diabete di tipo 1, e quello sono io.
Questo è un messaggio per chi, come me, convive con qualche problema di salute: non rinunciate ai vostri sogni!
È più difficile, affrontiamo più variabili e rischi, ma possiamo comunque raggiungere traguardi incredibili!

Il mio percorso non è ancora finito, continuerò a correre e a lavorare per l'inclusione delle persone con diabete di tipo 1 💙

PS: di solito non condivido questi dati, ma ho verificato i tempi complessivi più veloci per i 101 italiani e per i 1994 al mondo. Risulto 1 italiano e nella top 15 al mondo 👀

Every Marathon and every medal tells a story. 
The 2026 @capetownmarathon was definitively something special, like nothing I have done before. It felt like being part of something unique and special: an historic moment for the running world.
The race's claim, even engraved on the medal, was "Towards Africa's First Major" and all were united for this goal.
In fact, should this year's event pass the final assessment it would be recognized as the 8th @wmmajors , the first on African Soil, and all the ones who ran it to make this happen would be awarded with their star (already on our profiles as provisional).
The race surprised me in many ways. It definitively showed it is ready to be awarded the "major" status. Everything was super well organized and volunteers were super helpfull and willng to help.
The course is harsh, one of the toughest of the Majors, a relentless constant uphill that gives you no break, but It definitively has a unique vibe, something really african I never witnessed before.
The crowd will cheer you with colorfull costumes, tribal music and dances, you pass through people making barbecues and all the contradictions of this country with Table Mountain, Lion's Head and the sea to set a perfect scenery.
My own race was a real struggle: humidity at 100%, temperature has risen a lot during the race and I ended up having diharrea so badly that I had to stop 4/5 minutes in the potty just after the Half.
Then during the last 3km it came back, but I did not stop again, trying to make it to the finish line with one goal: run under the standard it was given me to obtain my 3rd @guinnessworldrecords, and I have made it.
I ended up super dehydrated despite drinking at all water stations and really tired.
The clock shows my worst time ever in a @wmmajors, before this one I had all the other 7 sub 2:50. Some may say it was a failure, but it is not.
Despite the harshness of the course and what happened, I still managed to handle my diabetes well, end in an amazing time and proving once again that it is possible to travel and perform in endurance sport with T1D.

And I will definitively be back!
