This has been a dream of mine since I started sketching anatomy drawings as a PT student. A goal I never thought would come to fruition, yet one that I am so excited to be sharing. I have always hoped to find ways to educate through pictures and art. Thank you to my colleagues who have provided me the space to transform these aspirations into reality.

Breast Cancer Series, Part 3: Micrometastatic Disease

Even after tumor resection with negative margins there is a chance cancer cells already migrated to other areas of the body. These cells are too small/few to appear on imaging as metastatic disease. With certain oncotype risk scores, adjuvant chemotherapy can be an important part of treatment to ⬇️ risk of recurrence and growth of metastases.

Special thank you to Dr. Tara Sanft, MD at Smilow Cancer Hospital for explaining this concept to me. I would see the term in documentation and also be confused if this was metastatic disease. The word “metastatic” is frightening, no matter the context it is given in. This graphic was made with the purpose of assisting patients in understanding their recommended treatment course and the meaning of this concept.

Breast Cancer Series, Part 2: Pectoralis Major Tightness Secondary to Tissue Expander Positioning

Following surgical removal of breast tissue, many patients opt to pursue reconstruction with breast implants. For many, this means tissue expanders must be placed in order to accommodate space for permanent implants.

Anatomically, breast tissue lies superficial to the muscles of the anterior chest wall. When expanders are placed, the positioning of pectoralis major is altered. Instead of sitting beneath the breast, pectoralis major is placed over the expander. Thus, as saline is injected into the expander with each subsequent fill, the pectoralis major muscle gets stretched to greater and greater lengths. The cumulative effects of this tissue stretch can result in significant pectoralis tightness and associated pain.

How can we help??
- chest opening exercises
- manual therapy for tissue mobilization
- *patient education*

Breast Cancer Series, Part 1: What is Radiation Fibrosis??

As radiation works to destroy cancer cells, it can also damage non-cancerous cells that exist within the irradiation field. Over time, this changes the cellular matrix and alters the structure of collagen in the tissues, resulting in fibrotic tissue sclerosis. This process is progressive and cumulative, oftentimes resulting in delayed symptom onset.

For individuals with breast cancer, radiation fibrosis may result in significant reductions in shoulder mobility, pain, tightness, muscle loss, skin sensitivity and axillary lymphatic cording. Now take a minute to appreciate the discomfort that these individuals may experience when lying with their arm positioned overhead for subsequent radiation treatments.

This is why early mobility is so important and why a consistent stretching regimen is imperative. Physical therapy is a resource all oncology patients should have access to.

Please feel free to share this graphic with family, friends and patients you know, going through radiation treatment. We have an obligation to empower our patients through education and knowledge.
