Pain often resolves before tissue recovery is complete. 

When pain settles, it means the nervous system is no longer signalling the same level of threat. It does not necessarily mean the tissue has regained its full capacity.

For example with tendon injuries, symptoms may improve within four to six weeks, while restoring the tendon’s strength and tolerance to load can take twelve weeks or considerably longer. It may feel normal before it is ready for unrestricted demand.

This is why pain alone is not a reliable measure of readiness. We assess strength, symmetry, movement quality, tolerance under load and critically; how the body responds in the 24 hours after training.

The question is not, “Does it hurt?”
But instead: “Can this tissue tolerate what you are about to ask of it?

If your training has plateaued, here is what your physiotherapist should be asking you: 

Sleep: How many hours are you sleeping a night, and how is the quality of that sleep?
Athletes sleeping under 7 hours show significantly reduced muscle recovery and up to a 60% higher risk of injury compared to those getting 8 or more.

Nutrition: Are you eating enough to fuel the training you are doing right now?
Muscle protein synthesis drops within hours of inadequate intake, meaning under fuelling for even a few days can stall strength and endurance gains regardless of how hard you train.

Recovery days: Are you actually taking them, or are they the first thing you skip?
Without at least one to two full rest days a week, the body cannot complete the adaptation process that turns training stress into actual fitness gains.

HRV and stress levels: What is happening with your nervous system outside the gym?
Chronically low HRV is one of the earliest measurable markers of overtraining, often showing up weeks before performance decline becomes obvious. Most of us collect the date but don’t know how to interpret it into their weekly schedule. 

Training plan: How is it structured? When are your rest days, and how are your strength days spaced against your endurance days?

The order and spacing of training days directly affects the quality of each session. Strength work on already fatigued legs, or endurance sessions placed too close to heavy lifting, means you are training hard without actually building anything.

It is running season and we are seeing a lot of running injuries in clinic.

One of the things we notice consistently is that most people either do not warm up at all or the warm up they are doing is not preparing the body properly ( we are talking to you desk based workers who sit all day and then decide to go for a run)!!

A warm up done well is what allows your body to perform at its best and stay injury free over time. Research shows that dynamic movement before a run reduces muscle stiffness, improves neuromuscular control, running efficiency, and increases your tissue and joint range of motion. 

Most people still do a static stretching warmup which does not achieve the same neuromuscular readiness and evidence suggests it can actually reduce muscle force and power. 

Here are a few of the exercises we prescribe at Swiss Touch before a run. We would love to know if you give them a try.

I had the absolute pleasure of attending @sarahannmacklin book launch last week and triaging guests at her book signing last night, and what a celebration both have been! 

Sarah has built her whole approach around asking why someone is there in the first place, not just handing someone a 30 day plan but understanding the huge importance of both psychological and physical presentation, and why self compassion makes the difference between short term change and lasting health. It is such an important message and one she delivers in a way that feels completely accessible (I have not been able to put her book down)! 

What I was reminded of last night triaging her guests is just how desperately people are looking for that kind of knowledge and education. A few examples of people I saw: a shoulder that had been painful for two years without a single professional opinion, six months of back pain with no real direction, a young woman with hypermobility who had never had it named or explained to her. I was able to provide an assessment, diagnosis, exercise prescription for both symptom relief and prevention, and perhaps most importantly empower the guests to help themselves. 

What Sarah's book does is exactly that. It gives people access to the right knowledge and will empower so many people to make better decisions about their long-term health. 

Sarah, I am so proud of how many people this book is going to help, and it was inspiring to see this firsthand. Go get the book, I know you are going to love it!
