⚙️ More Digi-Logue ⚙️

Lower free end saddle chromes are another classic example of the hybrid workflow - combining analogue principles with digital elements to get an optimal result!

I’m a big fan of the altered cast technique - a method of allowing you to balance the support from the hard and soft tissue - excellent for these free end saddles

This is done by taking an impression within the saddles on the metal framework - compressing the soft tissue with the framework at maximum engagement, just like it will be in function

This has the added benefit of also giving us a functional impression but also letting us make our framework off the scan and we love our scanning accuracy for the teeth normally!

My workflow here is as follows:

1️⃣ Analogue ‘built’ primary - I still like an impression here to guide the lab with where they should extend the custom trays to later - you need a model to plan the denture on so it may as well give us soft tissue markers too!

2️⃣ Scanned ‘secondary’ - after relevant tooth preps, in this case rests on the fours and composite cingulum rests on 3-3, we scan aiming for great detail on the teeth and as much length of the ridge as possible

The length of the ridge is important as the chrome will be made on this model and we need a good length of the metal frame over the ridge to support our saddles

3️⃣ Verification - here we have a printed verifier, great for patient consent about where metal may show but also if this fits then your expensive metal should too! If it doesn’t, re scan 🔁

4️⃣ Metal try and altered cast - the lab makes the metal work and adds trays to the saddles, once the fit is confirmed we can carry out the altered cast impression

- light body silicone with no adhesive to check fit, mark shine through with a pencil and adjust. Remember to engage the rest seats with finger pressure and not occlusion

- once adjusted add adhesive and repeated the impressions, with finger pressure, and border moulding

- the lab then sections the model & pours in the stone

5️⃣6️⃣7️⃣8️⃣ 🕺🏻 from this point you can carry on with your try in and your fit

💻 Digital Dentures 💻

This was one of our first digital cases over 4 years ago, it was great to see it published in ‘Digital Removable Dentures: A True Metamorphosis’

In this case we utilised the reference denture technique to make a new denture as efficiently as possible

This case was to replace an immediate denture where we were happy with the aesthetics, so the reference denture was essentially a reline and tweaking the occlusion

Think of the reference denture as the modern copy

With a reline in both the upper and lower dentures and scanning these 360 and in occlusion we could design the printed try in from the first slide

The occlusion (as it generally is with these digital cases) was spot on!

The great thing with the printed try ins is the patient can take them away for a better test run!

The reference denture is a super powerful tool I use regularly to improve efficiency in our workflows, particularly for @nick_fahey_prosthodontist implant work flows - we’ve got some cool cases coming!

I’m looking forward to teaching this in a couple of weeks on our digital denture course and internationally with a couple of courses in Europe and further afield in the pipe line 👀

If you’d like to check out the book it is available on Quintessence and to celebrate the launch I have a discount code so get in touch!
