$This is a young female that fell at work and suffered a Schatzker II Tibial plateau fracture. Soft tissues were amenable to definitive fixation. A standard anterolateral approach was done with submeniscal arthrotomy. The articular surface was elevated anatomically and the defect was filled with synthetic bone graft. We then closed the book and applied the plate. We allowed full motion immediately. She was allowed full WB at 6 wks. This was back with the old plating system from Arthrex. $How distal is too distal to nail and a tibia fracture? With modern nails, the screws have been placed far closer to the tip of the nail, which allows for a much more distal fractures to get nailed. The Arthrex nail has 3 screws within 21mm of the tip of the nail. This is a 59-year-old gentleman who injured his right leg. I am taking a CT for every spiral or oblique distal tibia fracture. We nailed his tibia and retrograde nailed his fibula the following day. He was allowed weight-bearing as tolerated at two weeks in a boot. $I’ve been nailing a lot more of these ankle fractures. Patients seem to be happier. Much less incision to close, and less to heal. I still open the fracture and clamp it. DO NOT SACRIFICE THE REDUCTION. $This is a female in her 60s who tripped and fell and injured her ankle. She was initially treated with a close reduction. At a week out from injury I performed ORIF. she was prone. I began with an MPM approach and fixated both segments of her comminuted distal tibia. I restored all articular impaction then used 2 mini frag plates. The fibula was then open reduced, clamped, and nailed with the Arthrex fibulock nail. Today she’s 6 weeks out and ambulating pain free with excellent ROM. I have found that since I left the posterolateral approach in favor of the MPM, my patients are far happier. I’ve had much better visualization of the posterior tibia. They have had far less stiffness in their ankle and toes. This has probably been the biggest change in my practice in the last 5 years. Well, that and I nail almost every fibula now. If anyone is still doing posterolateral approaches I highly recommend checking out Lyndon Mason’s presentation on you tube or any other platform going over the MPM. $Late 20s female who injured her ankle about a week before seeing me. On imaging if you look close, you can start to see a double bubble sign on the medial distal tibia. This should immediately tell you that a CT scan is needed for posterior articular evaluation. This is the posterior Pilon variant ankle fracture. Many surgeons will do this supine through a modified posteromedial approach and a direct lateral approach. Personally, I have a very difficult time with that. I have a hard time getting visualization and maintaining fracture reduction. I’ve can go much faster and much easier than the prone position. I start with the modified posteromedial approach. The first thing that gets reduced is the posteromedial component. We then reduced the posterolateral component down to it. Then will fixate them each independently. I don’t fixate the fibula first because that can block the articular visualization on the fluoroscopy. To get the true medium Malleolar component, I extend the incision a bit anteriorly from the distal pole. And then reduce and fixate the medial malleolus. I end with an open reduction of the fibula and fibular nailing.. The ankle was stressed under fluoroscopy, and the syndesmosis remains stable. She was allowed to weight bears tolerated at two weeks. She was essentially pain-free by six weeks. Due to some social issues, she never got to physical therapy. Both of these things have been a game changer in my practice. I have found the MPM approach to be much better than the PL. The incision heals much easier. There is much less soft tissue dissection. There is less stiffness in the ankle postop. I have also had far less Wound issues with very small incision fibular nailing than I did with open reduction and plates on a little malleolus. there is zero patients saying that they can feel their hardware. I definitely think there is something to not having a subcutaneous implant with an incision directly over it. How is everyone else handling this case? Positioning? Implants? Order of fixation? $Mid 80s active female with the standard slip and fall at home. Fixed within 6hrs of injury. She was up and walking in her room less than 24hrs after the injury. By 6 weeks she was no longer needing the walker but carried it for security purposes. Get these patients to the OR. Get the fixed and let them walk! Never let them sit over 24hrs (unless significant medial reasons have made you). Never make them partial WB. I’m enjoying this new nail that I’ve switched to. A telescoping lag screw that won’t allow lateral protrusion with fracture collapse. $Mid 60s super morbidly obese patient injured her humerus a few months back. Normally I will offer bracing of that fracture and they heal great. Unfortunately the soft tissue envelope being so large made it near impossible to control the alignment. I’m sure many of docs around the world would have nailed this. I don’t do a lot of humeral nails. I’ve revised a bunch due to nonunion, but not super common for me to nail a humeral diaphysis. Mipo plating would be another option. But nailing and Mipo are both made more difficult with massive soft tissues. So I did an ORIF with lag screws using an @arthrex_inc Alpha plate. With the plates that turn anterior like this, there is better preservation of the deltoid insertion and less risk to the radial nerve distally. She was allowed immediate ROM and allowed to fully WB with her arm for mobility.
