Sealing freshly cut dentin before beginning the indirect workflow may improve bond development, reduce postoperative sensitivity, protect the adhesive interface and preserve the substrate from contamination. In this publication, science, clinical experience and biological reasoning converge to explore immediate dentin sealing with two-bottle self-etch adhesives. We focus on two systems: • Clearfil SE Bond 2—the classic two-step self-etch adhesive.
• G2-BOND Universal—a two-bottle universal adhesive that, according to the available evidence, we recommend using in self-etch mode on dentin. Although their clinical sequences may appear similar, their chemistry and architecture are not identical. Consequently, each step—surface preparation, primer application, solvent evaporation, hydrophobic bond application, light curing and resin coating—must be adapted to the individual system. The goal is not to impose one universal protocol, but to optimize each material according to its composition, manufacturer’s instructions and the best evidence currently available. Similar sequence. LR6 Overlay 🌟 With cases like this, where cusps have fractured, and there is large amalgam restorations, a lot of cracks are expected in the tooth. The remaining walls ended up being very thin, and had many oblique cracks requiring the walls to be reduced. Usual biomimetic overlay protocols followed and restored using lithium disilicate with layering. Very unique dental anatomy and 3 years of treatment.  First we restored premolars in 2023, then the first molar in 2024, then we missed one year of recall and now it's 2026. Already in 2023 I said that the amalgam is causing cracks and even if the x-ray is fine for the second molar, I would still recommend the amalgam to be replaced. Well... 3 years later and now we have caries that also involved distal side of 1st molar. I am definitely not a fan of large amalgams (especially deep ones). What are the boundaries of adhesive dentistry? And.. Are you ready to push them further? Splinted crowns re-treatment with endodontic revision. You can clearly see from pre-op situation that the 17 abutment is living outside crown boundaries absolutely rent free. Isolation at stage of crown removal was quite challenging for a two-handed operator. IPDS and free-hand build-ups for both molars followed by endodontic re-treatment. I’ve made a decision to use all the enamel leftovers underneath and perform chamfer preps, followed by fabricating two e.max crowns by @tinatomova Glad it turned out this way! 🫡

Single visit full cusp coverage and vertical dimension increase. All restorations were replaced and occlusion raised by 3mm (including upper jaw) using direct compositerestorations. All done freehand following digital wax-up and re-scanned post every visit to evaluate accuracy and areas where corrections are needed. Timing: 4h10min each side. With a break in between. P.s. I know the matrix in the photo is not perfect, had to fix that in real life later. But given how complex the case was I had less time for photos. That feeling when you finalize a case that you worked hard on in the end of the week… Priceless! You might remember this case from a while back (The Double Dam case) of twin crown reconstruction. After we did a lot of work also on the adjacent teeth - endo retreatment and restorations, I was able to finalize the case with 2 homemade monolithic zirconia crowns. Happy to see the soft tissues in perfect condition and that nice polished collar of the crowns, combined with excellent fit, proximal and occlusal contacts!
