Do you POCUS? 🫀

Im very fortunate and at my institution I get to practice at the top of my license. Learning how to correctly do a cardiac point of care ultrasound (POCUS) is my personally favorite skill set I have.

Why? Yes I still order formal echos, but far too many times a quick look is all we need. To avoid added cost and over-utilization of resources and trouble shoot emergencies.

👇🏻This is a cardiogenic shock patient who was placed on peripheral ECMO but due to anatomy unable to have a LV vent placed. A big known complication from this can be a developing thrombi. To ensure it’s not occurring I perform serial POCUS on this type of patients in addition to ensuring they have a chemical vent (inotrope). And monitoring for good pulsitility.

20/10 recommend all CVICU apps to learn this skill well. Itll make you a great asset for your practice and patients ❣️

I work in a trauma 1 level center where we care for the sickest, our unit has ECMOs, impellas galore, pre transplant, LVAD, cardiogenic shock and advanced heart failure.

It feels like yesterday when we were taking care of afib, and STEMIS.

‼️Patients are getting sicker and younger - so check your labs people and take your cholesterol pill 💊

‼️Age doesn’t discriminate anymore with cardiovascular disease.

⚡️POCUS GEL: SYRINGE APPLICATOR

The quality of ultrasound imaging heavily depends on the appropriate transmission gel and its placement.

The typical gel bottles with a squeeze-type dispenser mechanism are slow and tedious to use, leave large volumes of gel unused and are usually large and bulky.

How to mitigate those issues and make the gel more user-friendly at the POI? Pre-filling it in syringes.

⚡️PREPARATION:

1) Take a Luer Lock syringe. We recommend a 20 mL syringe size for a classic eFAST exam.

2) Take out the plunger and fill the syringe from the rear.

3) Reattach the plunger and squeeze out any air bubbles.

4) Apply a Luer-Lock cap.

5) CRITICAL: Visibly mark syringe as USG Gel! Do not store with medications to avoid medical errors!

⚡️USAGE:

- Using the syringe, pre-apply gel to all POCUS viewing windows before starting the examination.

- One POCUS view requires approx. 2-3 mL of gel, so one eFAST exam (6 views) requires between 10-18 mL of gel.

- Perform POCUS exam.

- Discard the syringe.

Keep those ready to go in your chest rig or med bag.
Fire and forget at the point of care.

I’m going start a series about ultrasound basics and how to approach POCUS for everybody.

Which probe you decide to use dictates the quality of your image. The linear probe is high frequency, which gives you great detail, but only to about 6 cm so it’s a great jack of all trades probe. The curvilinear probe has lower frequency but penetrate deeper so you’ll get worse resolution but a wider field of view which is really good for looking into your abdominal space. The phase array is also low frequency, but because it has a smaller footprint, it can go between ribs, which is why we call it the cardiac probe.

The higher, the frequency, the better the image. Lower frequency gives you more depth with a slightly worse picture. Using the @butterflynetwork scanner allows you to quickly come through different presets with one probe so you don’t have to make the decision yourself!

The thyroid and carotid view is one of the first neck windows that’s worth learning even for emergency medicine. The scan can help you look at airway landmarks in a neck that has obesity, swelling, trauma, subcutaneous emphysema that may obstruct palpating the cricothyroid membrane.

Additionally, something that’s not shown in this video, you could use a transtracheal view above the sternal notch to see whether or not your endotracheal tube is in the trachea or the esophagus. You could also look at how close any neck masses or hematoma are to the parent vessels (common carotid and internal jugular).
