Residents were highly engaged with @apromisetocare_east @aptce_foundation & @aptce_allofus Medicaid expert @djshortyfatz —and called him that same day! We secured TWO referrals with case manager contact information to help move the homecare process forward. 🖤♥️🤍

A special thank-you to our Community Engagement Director, Joi Tate @jct_xtraaf the Mayor of @citymaplehts , and the city’s Human Services Department for connecting us with the service coordinator who helped pave the way for our last THREE Senior Tour stops! 👏🏽
