People aren’t failing to prioritize their health.

They’re being forced to ration it.

Today, a woman cried to me because after paying hundreds of dollars every month for health insurance, she still couldn’t afford the copay to see a doctor.

Moments like this are part of why cash-pay medicine seemed so attractive to me—and why I opened a cash-pay practice.

I wanted patients to know the price before saying yes. I wanted 60 minutes to listen instead of rushing through a visit. I wanted fewer surprise bills and fewer decisions shaped by what insurance would reimburse.

But here is the uncomfortable truth: cash pay is not affordable for everyone either.

Transparency matters. Time matters. Freedom from insurance barriers matters. But none of those automatically creates access.

There is no perfect payment model inside a system where even insured people cannot afford care.

I still believe patients deserve transparent pricing and enough time with their physician. I also believe no one should pay hundreds every month for insurance—and then cry because they cannot afford to use it.

What would make healthcare feel truly accessible to you?

I keep saying I’m staying out of politics but it’s getting impossible. Healthcare and politics are intertwined when it comes to system change. I’ve got a lot of workarounds for the current system but they are just that- workarounds. We need policy change to really move the needle. A love letter to the original preventative medicine doctors.

Long before ā€œlongevityā€ and ā€œoptimizationā€ became wellness buzzwords, primary care physicians were already preaching the real foundations: exercise, nutrition, sleep, and stress reduction.

But here’s the heartbreak — in insurance-based practices, the average visit is just 15 minutes, often covering 6 separate health concerns. That’s about ONE minute per problem. And research shows a PCP would need 27 hours in a day to deliver all the guideline-recommended care for a typical panel.

This was never a failure of your physician’s expertise. It was a failure of time.

So when you feel rushed and unheard, the answer isn’t trading down to a less-qualified, non-medical ā€œwellnessā€ provider.

The answer is more time with a real physician.

In my new piece, I make the case for Direct Primary Care — a model that gives patients their doctor back: longer visits, direct access, and care built on relationship instead of a stopwatch.

The data backs it up — twice the face time, fewer ER visits, and 99% patient satisfaction in one program.

To every primary care doctor: you were never the problem. Patients don’t need less of you. They need more. šŸ’›
