That feeling when you finalize a case that you worked hard on in the end of the week… Priceless!

You might remember this case from a while back (The Double Dam case) of twin crown reconstruction. After we did a lot of work also on the adjacent teeth - endo retreatment and restorations, I was able to finalize the case with 2 homemade monolithic zirconia crowns. Happy to see the soft tissues in perfect condition and that nice polished collar of the crowns, combined with excellent fit, proximal and occlusal contacts!

LEGO 2.0 / Printed Composite Build-Up and Temporary Crown

I think this will be a far more accessible option to manufacture this kind of adhesive crown build-ups and long term temporary crowns.

If you know me then you know my unhealthy obsession with proximal contacts. Let me break it down.

Separation. Separation. Separation. You will have to compensate the thickness of your matrix - the thicker the matrix, the more separation you will need. Wooden wedges provide a much greater separation than plastic/silicone ones and can be helpful especially when inserted in the beginning of the procedure (pre-wedging) as they are easily customized with the preparation to not interfere with the matrix placement pushing it away from the contact surface. Certain proximal rings also provide great separation - again the stronger the ring, the greater the separation. A strong ring can be used with a plastic wedge (Garrison/Bioclear), a weaker one needs to be combined with a wooden wedges (Custom Ring).

Marginal Ridge. Make it thick. The thicker, the better. Just take some time to observe the marginal ridge anatomy of a healthy tooth and notice the distance you have from the peak of the ridge to the contact surface (usually 1-1.5mm) creating the important occlusal embrasure. This is a common mistake to make the highest point of the marginal ridge too close to the proximal contact surface and abolishing the opportunity to create a proper marginal ridge and embrasure.

In the end, great proximal contacts are created with intention, not luck. After all of our hard work, we are still not done. Take the time to separate the teeth even further once you have finished the restoration and observe what is between the teeth. Honestly the first time I saw that I felt bad for every Class II I have done. There is always excess and residue there, so a Elliot/Ivory separator or a bigger wooden wedge can help us access this area and finish the surface without weakening the contact.

Hope some of this is helpful, as always I would love to see your take and questions on this topic in the comments!

You want to have a smooth experience with your next Class V restoration? This one is for you!

Now let me tell you, over the years I have had my fair share of struggles with Class V restorations, especially with unwanted residues, clean up and finishing, so here are some tips and tricks that can really help you deliver a good cervical restoration!

First things first - proper rubber dam isolation. This one is so crucial so don’t even get me started. I like to place accessory clamps (B4/106) that grasp at least 1mm under the margin of the defect (can be a bit less in extreme cases). Adding teflon tape in the sulcus is also essential and make sure you get that much needed horizontal retraction and isolation (this will be key later on).

I like to imply a “No Burs Allowed” rule for my Class V’s so talking preparation I just use AquaCare for air abrasion and cleaning of the surface with Aluminum Oxide. Easy as that, takes just a few seconds to have the perfect surface without further injury/removal of tooth structure.

Here’s the big (or should I say small) game changer for me - the microsuction / endo-aspiration. This little fella is so helpful to suck up all the residue of adhesive / bonding on the teflon tape and clamp leaving you with little to no excess to clean up. Once you try it you will not go back! I use it right before we polymerize our adhesive system.

All of this facilitates the finishing and polishing procedures, making them fast, easy and efficient. Remember, no burs, just use a composite scaler / 12D blade to take care of those last little excesses of adhesive or composite and you are golden!

If in doubt let the soft tissues do the talking! Last pic is the final result after just 2 days.

If that was helpful please leave a comment or repost the case! That would be much appreciated! 🙏🙏

Another amazing opportunity to travel around the World and share my passion, knowledge and experience in the field of adhesive restorative dentistry and spread the message of the Bio-Emulation approach. This time the journey has led me to the distant lands of India where I spent a week and visited a few cities and events to talk about indirect and direct adhesive restorations, digital dentistry and dental photography.

The main event of course was a 35-year old congress, a teal trademark of my dear Indian colleagues, The Indian Academy of Aesthetic and Cosmetic Dentistry in the state of Goa. I was impressed by the high level of organization and professionalism in every detail. Thank you very much for making me a part of this wonderful event filled with great lectures, workshops and hands-on courses.

Special thanks to my friends @swabhaanu_sindagi and @dr_deepak_mehta for the invitation and for making me feel at home so far away from it! I am glad I got to hang out with you guys, not only both of you are exceptional clinicians but you sure do know how to have some proper fun too! I had the opportunity to meet with @drcalinpop and even if I couldn’t attend his lecture it was nice to spend some time together!

Thanks also to the colleagues at GC India and miss Anjali in personal for taking care of me and for organizing my lectures and workshops in Bangalore and Mumbai.

As always the best part of this trips and appearances is to represent my homeland of Bulgaria and to share about my team, friends and loved ones!

The Journey is just beginning!
