reminder that we have until October 22 to review and submit comments to ACGME regarding their proposed changes to residency requirements!

SHARE to your fellow or incoming residents/fellows + attendings who still care about a better future!!!

the INFURIATING ones:
• 6.30, pg 72 removal of “residents should have 8 hrs off between scheduled clinical work and educational periods” due to assuming it’s an erroneous detail given 80 hr work week avg restriction
• 6.31 pg 72: reducing required time off after 24 hr call shift from 14 hrs to 12 hrs —less protected post-call time for residents
• 6.38 pg 77: remove “maximum in house on call frequency” that residents cannot be scheduled for inhouse call more frequently than every 3rd night —now allows programs to require inhouse call every other night if doesn’t break 80 hr avg limit

the beneficial ones:
• removing “educating residents about fatigue and sleep deprivation, noting this has created a checkbox exercise without additional benefit” —as a resident this talk was like a slap to the face, so good riddance
• requiring at home call to count to 80 hr max weekly

we are already bending over backwards and skipping bathroom breaks, lunches, sleep, & time to prevent burnout to provide compassionate empathetic care to patients. meanwhile, the internet & others are projecting their frustrations on the healthcare system onto the clinicians trying to advocate for change for their patients. residents arguably feel the brunt of this more than attendings who have more autonomy.

so why, ACGME, propose these changes that can allow worse scheduling practices for residents if you claim your changes are to “decrease burnout” and “ensure adequate sleep”??? if a hospital can’t function without the labor of severely underpaid, sleep deprived residents then address the institution NOT punish vulnerable.

have I ever made espresso before? no, but I’m about to learn!!! 😅 seriously, please give me tips on what other espresso tools are immediate “must haves” (like a bottomless portafilter? springloaded tamp? knock box?) vs “nice to haves.” also where you like to get espresso cups from and are they really necessary or can I use a teacup?
ideas for optimizing the little space of the coffee nook for both function & aesthetic? reliable brands?

everyone’s been asking us recent graduates “what’s your first attending-level purchase going to be?” and this is mine! I spent a couple weeks figuring out what machine would fit my lifestyle & noob espresso-making status the best —finally settled on the @breville the Bambino Plus for its super quick heat up & auto milk frothing capabilities + @fellowproducts Opus 2 conical burr grinder for both espresso & cold brew. huge thanks to friends who answered my questions too like @andy_nguyen9 & blogs like @ukcoffeeblog @bonappetitmag for their rankings.

if you’re an attending, what was your first big purchase?

The hidden curriculum of medicine’s tentacles extend into the residency interview. The good news is that there are many ways for med students to set themselves up for success as interview season approaches, and we’ll talk all about those: aka how to be a green flag. While every interviewer may be looking for something slightly different, there’s an art to avoiding being remembered on the interview trail for the *wrong reasons.* We’ll warn you about them! Join us for this free webinar on October 5 (link in bio). There are no stupid questions, bring them all!

Ex Vivo exists in the space where identities meet. The clinician and the artist, the trainee and the storyteller, the healer and the humanist. This issue asks contributors to sit in that overlap on purpose: not “medicine, and also art,” but the place where the two actually touch and reshape each other. We’re looking for work that explores Intersections: the moments where your medical training and your creative practice collide, combine, or redefine one another.

sometimes you just need to sit in silence to process the crazy or super heavy things you witnessed, but didn’t have time to think about in the moment bc you had to move onto the next patient

if you need to vent, comment a wild moment (without violating hipaa) to let it out and process. more people might be able to relate than you think!

anyone else get home from work and tend to just sit in your car in silence, staring into space for 30 minutes or until your husband or roommate pops their head out and asks “…what’re you doing?”?

it’s not even a matter of introverted vs extroverted (although the threshold is much shorter for introverted). i don’t think people realize how much energy it takes to be your most friendly, empathetic self to 20+ strangers while thinking critically to figure out how to treat them. add on talking for 6-7 hours total, and silence becomes verrryyy welcoming

what many patients (pts) don’t realize is that in traditional models for clinic, appointment slots are back to back 10-20 minutes per patient (pt) with maybe a 30-60 min break for lunch & 60 min for admin (reading + addressing results, finishing notes, answering messages from patients and consultants, inbasket) in a day. most of us are spending hours outside our paid time to get all these tasks done to do everything we can to take care of you.

when the schedule is full, here’s what would need to happen to stay on time:
• pt arrives BEFORE appt time, and check-in and rooming process is seamless so that pt is ready to be seen by physician at time of appt
• chief complaint is 1 or 2 issues that are straight forward. if they’re complicated, the physician already knows the pt’s history well to quickly come up with a diagnosis & plan that aligns with the pt
• some education is given, but doesn’t need much
• MA and physician set up & can do procedures efficiently
• physician has an efficient system to complete notes during the visit

how the schedule can be derailed quickly:
• pt arrives 10 minutes late so by the time they’re roomed, their appt slot is over
• inefficient check-in and rooming process
• physician runs late bc pt’s issue was more complicated than expected. for example: new diagnosis diabetes, mental crisis, uncontrolled severe blood pressure, need to send to the ER, procedure not going as planned. bonus when there’s multiple pts needing elevated care
• physician tries to do too much in the appt (address 3+ issues, offer recommended screenings, takes time for educating pt, etc) bc they care

the schedule tends to get derailed bc we’re human. so when your physician asks you to come back for another appt to spend time on your other concern rather than rushing, it’s bc we care, not bc we want more money from you. most of us physicians just want to practice medicine and wish we had extra time with patients to try to fix all their health concerns. we’d rather not worry about the admin, systemic, or political issues that bleed into our job & burn us out, but we have to.

i’m actually thriving and not just surviving?!

i think i can speak for most people in the same position when i say: THANK GOODNESS RESIDENCY IS OVER. very grateful to have matched to a residency program and for all the growth during the past few years…but hopefully we never have to relive those traumatic experiences or work 76 hours a week while getting paid $12.50/hr ever again.

Huge congratulations to Dr. Mark Fraser, MD, PhD, on his recent publication in The New England Journal of Medicine!

It’s not every day that one of our fellows publishes in NEJM, and we are incredibly proud of Mark for this outstanding accomplishment. His piece, written with Dr. Laura Vater from the Department of Hematology and Oncology and colleagues, highlights the value of fast-track pathways in internal medicine residency for trainees pursuing subspecialty training and careers as physician-scientists.

This is a remarkable achievement and a great example of the thoughtful scholarship our fellows are contributing to medical education and the future of physician-scientist training.

Congratulations, Mark!
