Schistocytes are fragments of red blood cells that can be found in a peripheral blood smear . When detected, it is important to promptly review and investigate for thrombotic microangiopathy (TMA) according to laboratory policies, as it is considered a hematological emergency. However, it’s worth noting that schistocytes are not exclusive to TMA and can be observed in other conditions as well. The fragmentation of RBCs can occur due to mechanical damage in the circulation and may be seen in patients with mechanical heart valves or those undergoing dialysis. Furthermore, schistocytes can also be present in acquired and inherited RBC disorders with significant anisopoikilocytosis, which affects the cytoskeletal structure of RBCs.

BASIC REVIEW OF ACID BASE.
First, look at the pH. Below 7.35 is ACIDOTIC. Above 7.45 is ALKALOTIC. Then look at the CO2. CO2 is an acid, so as CO2 levels go up, pH goes down. Normal CO2 is 35-45. If the CO2 is driving the change in pH, we call it a respiratory issue. For example, if the pH is 7.25 and the CO2 if 55, we state that we have a RESPIRATORY ACIDOSIS. Next, look at the Bicarbonate level (HCO3). HCO3 is a base so as the HCO3 levels go down the pH levels also go down. If the HCO3 levels are driving the change in pH we call it a metabolic issue. For example, if the pH is 7.25 and the HCO3 is 15, we state that there is a METABOLIC ACIDOSIS.
The next video will cover metabolic acidosis. We will review anion gap and the Winter’s formula to determine if we have appropriate respiratory compensation.

67 year old male comes in for left low back/flank pain, radiating into the groin and down the left leg . No trauma. No back pain red flags. No IV drug use. No other complaints. BP 179/106 otherwise stable. CT shows this…what’s your diagnosis and treatment?
