Had a very productive talk yesterday with @mariacarolinaestomaterapeuta and @dr.feraru about medicine/health apps Nina Foot Care™ and Wound Scribe. Topics discussed were diabetic foot, wound care and amputation. Glad to be part of that discussion!

🌎 Pediatric Flatfoot & Newborn Deformities — What Did We Actually Learn?
The 4.22.26 session wasn’t about “fixing feet.”
It was about understanding the child above the foot.
Let’s recap one of the most commonly misunderstood newborn conditions:
👶 Positional Congenital Calcaneal Valgus (CCV)
(aka: the foot that looks like it’s touching the shin)
________________________________________
🔑 KEY TAKEAWAYS
👉 It’s usually flexible
If you can plantarflex toward neutral → you’re in the right diagnosis
👉 It’s COMMON
• Up to 40% mild
• ~5% severe
• Often related to intrauterine positioning (think breech)
👉 The MOST important exam finding?
➡️ Ability to plantarflex toward 90°
Flexibility > appearance
👉 Don’t miss the bigger picture
“There is an entire newborn above that foot”
Always screen for:
• Hip dysplasia
• Knee instability
• Syndromic features
👉 Natural history is your best friend
💥 >99% resolve WITHOUT intervention
________________________________________
⚠️ When people get into trouble
Confusing CCV with:
• Congenital vertical talus (rigid rocker-bottom)
• Posteromedial tibial bowing
• Associated deformity syndromes
➡️ Find the apex. Check flexibility. Don’t guess.
________________________________________
🛠️ Treatment (when needed)
• Mild → reassurance ± stretching
• Moderate → stretching, serial casting, possible AFO / DAFO
• Severe → short course serial casting
🚫 No need for aggressive therapy
🚫 No need to panic parents
________________________________________
💡 What parents actually need to hear:
“This looks dramatic—but it almost always corrects on its own.”
________________________________________
Great discussion yesterday with an engaged audience—this is exactly how we prevent over-treatment and missed diagnoses.
If you missed it… more coming soon.

🚧 PEDIATRIC FLATFOOT… from the ground up 🚧
Ever feel like evaluating flatfoot is like this road sign?
⬅️➡️⬆️↙️↘️… good luck.
Don’t worry, we’re not going down every possible pathway.
We’ve got 1 hour, not a fellowship.
👉 Instead, we’re keeping it simple (and clinically useful):
✔️ Flexible flatfoot
✔️ With and without short Achilles
✔️ Real cases across growth stages
👶 Newborn
🧸 Toddler
🏃 School age (with & without equinus)
🧑‍⚕️ Adolescent flexible flatfoot with short Achilles (yes… the ones that actually need surgery)
💡 Goal: Clear thinking. Practical decision-making. Less “it depends…” and more “here’s what to do.”
👨‍⚕️ Joining me @helpkidswalkdoc is Dr. Lucian Feraru @dr.feraru
(Not pictured… for his safety during the making of this ad 😄)
⚠️ Dad humor disclaimer:
– Jokes: inevitable
– Puns: likely
– Every type of flatfoot: absolutely not covered (you’re welcome)
📅 April 22
⏱ 1 hour
🧠 Worth your time
Let’s simplify flatfoot… and maybe even enjoy it a little.

Najlepsze souveniry z zagranicznych staży to te w formie wspólnej publikacji naukowej.

The best souvenirs from international fellowships are those in the form of a joint scientific publication. During my fellowship in Los Angeles, Dr. David Armstrong not only made sure that I learned as much as possible, but also showed me great trust by inviting me to join a team working on the development of a chatbot that will assist patients with diabetic foot syndrome and adjust physical activity based on deviations in measured parameters. All of this is aimed at prolonging remission.

A healed wound does not mean a permanent cure—it means remission. It is essential to ensure that no new wound develops, and unfortunately, recurrence in diabetic foot syndrome occurs in as many as 40% of patients.

You can read more about this in our new article, which was published today in the journal “Sensors,” as well as on Dr. Armstrong’s blog.

Congratulations once again to Lucian Feraru and the entire team ❤️

Pediatric flatfoot is one of those presentations where the wrong call in either direction costs the patient. Treat everything and you're over-intervening. Miss the right surgical candidate and you're watching a preventable deformity progress.

Flatfoot Fails & Valentine's Arches: When to Love and When to Operate gives you the clinical framework to tell the difference. Dr. Mark Solomon @helpkidswalkdoc and Dr. Lucian Feraru @dr.feraru walk through developmental arch milestones, flexible vs. rigid flatfoot, tarsal coalition workup, and the real indications for calcaneal lengthening and coalition resection.

Wednesday, April 22 at 7:00 PM ET. Free and live.

From every internship I look for inspiration - something I can learn from others and bring back with me to Poland. One of those “things” will definitely be the shared celebration of the successes of the research team. Today we celebrated the enrollment of patients into the study that David’s team is involved in. It was wonderful to spend this Sunday with you and celebrate together. Thank you for having us @d.g.armstrong & Tanya ❤️🇺🇸🌸

Yesterday was particularly special, as I got to share a podcast with two of our three daughters, who joined Joseph Mills MD Tze-Woei Tan, MD, MPH Michael Conte, Rich Swanson on a manuscript in International Wound Journal as co-first authors. Hosted by the legendary Prof. Keith Harding, CBE. Life affirming stuff! #ActAgainstAmputation
