GUIDED SURGERY IS NOT THE GOAL.
PROSTHETICALLY GUIDED IMPLANT PLACEMENT IS.
If my objective is simply to place an implant in bone, I can raise a flap and place it freehand.
If my objective is to place the implant in the ideal prosthetic position, I use a surgical guide—whether static or dynamic.
But let’s be clear: the guide is not the surgeon.
In immediate implant cases, the guide gets me very close, but clinical reality often differs from the virtual plan. Bone quality, socket anatomy, and primary stability frequently require intraoperative adjustments.
One of the most common adjustments is implant depth. In my experience, the planned position is often slightly too coronal. During surgery, I regularly decide to place the implant a little deeper than originally planned to optimize the prosthetic outcome and soft tissue architecture.
This is why I don’t believe in blindly following a guide. I believe in prosthetically driven surgery supported by guidance and refined by clinical judgment.
The guide provides direction. The surgeon determines the final implant position.
💬 Do you always follow the planned depth, or do you also adjust it clinically in immediate implant cases?
