OPHTHALMIA NEONATORUM | DISEASES OF THE CONJUNCTIVA

🔴 Definition: Conjunctival inflammation occurring in a newborn during the first 28 days of life.

🦠 Common causes

* Neisseria gonorrhoeae: Usually presents 2–5 days after birth; profuse purulent discharge.
* Chlamydia trachomatis: Usually presents 5–14 days after birth; mucopurulent discharge and possible pneumonia.
* Herpes simplex virus (HSV): May cause keratoconjunctivitis and serious systemic disease.
* Chemical irritation: May occur after exposure to prophylactic agents.

👁️ Clinical features

* Redness and conjunctival swelling.
* Eyelid oedema.
* Watery, mucopurulent or profuse purulent discharge, depending on the cause.
* Corneal ulceration in severe gonococcal infection.

⚠️ Complications

* Corneal ulceration and perforation.
* Corneal scarring and blindness.
* Chlamydial pneumonia.
* Disseminated infection in severe cases.

💊 Management

* Suspected gonococcal infection: Emergency assessment and systemic ceftriaxone according to neonatal guidelines.
* Chlamydial infection: Systemic macrolide therapy; assess for associated pneumonia.
* Suspected HSV: Urgent specialist assessment and systemic acyclovir when indicated.
* Clean discharge gently with sterile saline and investigate the causative organism.
* Evaluate and treat maternal infection where appropriate.

🚨 Exam pearl: Ophthalmia neonatorum is a neonatal eye emergency when gonococcal infection is suspected. Topical treatment alone is inadequate for gonococcal or chlamydial infection.

Definition Ankylosing spondylitis (AS) is a chronic inflammatory disease that mainly affects the spine and sacroiliac joints (joints between the sacrum and pelvis). It causes back pain, stiffness, reduced spinal mobility and, in advanced cases, fusion of the vertebrae. ### 2\. Causes / Etiology The exact cause is unknown, but it is associated with genetic and immune-system factors. * HLA-B27: Strong genetic association. * Autoimmune-related inflammation: Immune-mediated inflammation plays an important role. * Family history: Risk increases if a close family member has the disease. ### 3\. Risk Factors * Age: Usually begins before 40–45 years. * Male sex: Historically diagnosed more often in males. * HLA-B27 positivity. * Family history of spondyloarthritis. * Smoking, which is associated with worse disease outcomes. ### 4\. Pathophysiology The disease process occurs as follows: * Inflammation develops at the entheses (sites where tendons and ligaments attach to bone) and in the sacroiliac joints. * Inflammation causes pain and stiffness. * Repeated inflammation may lead to new bone formation. * Vertebral bridges called syndesmophytes may develop. * In advanced cases, vertebrae may fuse, producing a characteristic bamboo spine appearance on X-ray. ### 5\. Signs and Symptoms * Chronic inflammatory back pain: Usually lasts more than three months. * Morning stiffness: Improves with movement and exercise. * Pain at night: May improve after getting up and moving. * Reduced spinal mobility: Difficulty bending the back. * Alternating buttock pain: Due to sacroiliac joint involvement. * Reduced chest expansion: Involvement of the joints between the ribs and spine. * Fatigue: Due to chronic inflammation. ### Extra-articular manifestations * Acute anterior uveitis (painful, red eye with blurred vision or light sensitivity). * Psoriasis. * Inflammatory bowel disease. * Rarely, aortic inflammation or regurgitation. #rehab #physiotherapy #medicalstudent #trending #fypage$
