Chronic pain in midlife – why are we talking about it?

From menstrual cramps and endometriosis to pregnancy-related pain and musculoskeletal symptoms in midlife, women may experience different forms of pain throughout their lives.

Yet women’s pain has historically been dismissed, normalised or attributed to being overly sensitive.

This year’s World Menopause Day theme, Lifestyle Medicine and Menopausal Pain, gives us an opportunity to look beyond hormones and explore the complex relationship between menopause, pain and the nervous system.

Not all pain is caused by tissue damage. Hormonal changes, sleep disruption, stress and changes in how the nervous system processes pain may all play a role.

Pain is real, even when we cannot see it.

It’s time to listen, understand and take women’s pain seriously.

As physicians, is it enough to only treat patients when they appear at our consulting rooms with symptoms or distress already staring them in their face? To wait till they realise that something could be wrong and then present themselves to us? How many cases get missed this way and how many people remain undiagnosed?
The numbers tell the story for themselves. We as a society are buckling under the burden of NCDs and metabolic disorders. Preventive medicine and Lifestyle medicine hold the answers to this burden of disease that we are facing routinely. It is also not nearly enough to just tell patients that ‘lifestyle’ needs to be changed. And preventive health is not just about doing six-monthly health packages, including full-body scans.
What does the individual do with all the data that they now have easy access to? How does that translate into everyday choices and changes which the patient can make to elevate his health?
I spent the weekend at the 7th International Lifestyle Medicine Conference and heard some very insightful talks. As a member of @lifestylemedicineindia and a soon to be a Diplomate of the ISLM, it has been just wonderful to learn in detail about all the evidence behind the things that I routinely counsel my patients about. The six pillars of Lifestyle medicine – nutrition, physical activity, mental health, sleep, healthy relationships and avoidance of substance abuse – seem so obvious, but how do we ensure that when our patient leaves our cabin with an advice of “lifestyle changes”, they actually know what to do next? And what role can we play in ensuring that they do?
Lifestyle Medicine is an evidence-based practice of assessing the health of an individual on the parameters of lifestyle pillars and then prescribing changes in them in an objective, easily followable way. While I wrote about this in my booklet Let’s Simplify Health to educate my patients, this course focuses on prescribing the evidence-based recommendations so that when the patient leaves our cabin, they know exactly what they need to do to.

What happens when you ask a movement coach to stand still for a photo.

You do not get a photo. You get a warm-up.

Darryl Edwards had just spent twenty minutes on stage making the case that exercise is treatment in oncology, not an add-on. Then he walked off and proved it in the foyer. First frame, we are almost behaving. Second frame, three grown men in suits are mid-lunge outside Café π at the Oxford Mathematical Institute, and none of us can remember who started it.

That is the point he makes better than anyone. Movement is not a prescription you write. It is something people are pulled into. If a foyer full of doctors at a conference cannot resist it, a patient with a bit of encouragement has a chance too.

Great to share a stage with you, Darryl. Even better to share a floor.

Saturday, the opening plenary on cardiometabolic health across the cancer pathway. My argument: a cancer diagnosis is the strongest teachable moment in medicine, and we are wasting it. Treatment pushes weight, insulin resistance and blood pressure the wrong way. Survivors of several common cancers are now more likely to die of heart disease than of recurrence. Prehabilitation in the window before surgery fixes both problems with one intervention.

Sunday, delivery. A fatigue workshop built on the 2024 ASCO-SIO guideline: exercise, CBT, mindfulness, tai chi, acupressure. Then a perioperative workshop on how to screen, prescribe and monitor prehabilitation in a real NHS pathway. Becky Hogan’s talk on nutrition and her own experience as a patient was the one the room will remember.

Integrative oncology is not an alternative to treatment. It is what makes treatment survivable.

What a week it was.

Monday and Tuesday in theatre, ICU and Foundation School work. Wednesday building lifestyle medicine modules.

Thursday at the House of Lords with IndiSetu. A long conversation with Lord Mervyn King on why prevention keeps losing the funding argument. Then the moment of the week: meeting Dame Parveen Kumar. Every doctor of my generation learned medicine from Kumar and Clark. She asked about my work. I will not forget it.

Friday morning, Bucharest, presenting online to the World Psycho-Oncology Conference. Friday afternoon, BMA House, on the IndiSetu panel on AI in preventive wellness. Detection is easy. Behaviour change is the hard part.

Saturday, the first conference of the Lifestyle Medicine Academy we founded this year. Faculty from the USA, UK and India teaching a cohort preparing for the IBLM diploma. Kochi follows in October.

Saturday and Sunday, the NCIO congress at Oxford’s Mathematical Institute. One lecture, three sessions chaired.

Speaking at the National Conference for Integrative Oncology (@ncio_uk) at @oxford_uni was such a meaningful experience.

The weekend was filled with wonderful education, nutritious food, superb company and thoughtful conversations.

The evidence for exercise with cancer is strong, but we also know that taking those first steps can feel incredibly challenging. That’s why joyful movement should be prioritised to make it easier to celebrate every step.
