Endodontic Retreatment | The Importance of Understanding Root Canal Anatomy

The main problems: Under-obturation of the distal canal system and a missed canal.

This case was referred to us due to failed endodontic treatment. Following careful radiographic examination, we identified that the failure was associated with the distal canal system, while the mesial canal system had been obturated almost perfectly.

retreatment protocol:

- Removal of the previous obturation material.
- Canal disinfection and cleaning using the FANTA retreatment kit.
- Final canal preparation using FANTA Sone Blue, size 25/.06 taper.

Take-home message:

Never underestimate the importance of taking multiple periapical radiographs with both mesial and distal horizontal angulations. Different projections can reveal hidden canals and provide a much clearer understanding of the complex root canal anatomy.

Now, notice how the anatomy of the root canal system changes in appearance between the mesial and distal angulations . This difference highlights why relying on a single radiograph may lead to missed anatomical details and compromised treatment outcomes.
