Mobility and rapid deployment are central to operational readiness.

The CTOMS Training Cell is currently breaking down and preparing for immediate relocation, all while seamlessly resetting for the next TacMed course.

Just as we train operators to adapt to fluid environments, our training infrastructure is built for maximum agility and manoeuvrability. We keep our footprint lean and our deployment fast to deliver critical Tactical Combat Casualty Care (TCCC) and advanced trauma management training wherever the mission requires.

Discover our mobile and scalable training solutions at CTOMS Mission Essential Training Programs.

THE GOLDEN HOUR 🕰️

In tactical medicine, our greatest enemy isn’t always the one standing across from us. Sometimes, it’s time itself.

Every second matters when a life hangs in the balance. From the first assessment to the final handoff, we’re fighting a race against the clock.

The Golden Hour isn’t just a concept taught in medicine.

It’s a reminder that every decision, every intervention, and every moment of hesitation can change someone’s outcome.

We don’t always get to choose when the clock starts. But we train relentlessly to be ready when it does.

One of our OG cohort bubbas recently cleared flight boards and we’d like to show him some love.

We provided him with the resources to succeed, he was the one that put in the work.

From HALO to Flight Provider. Quite the career thus far!

2027 is going to be huge for medics that get accepted into our program. Stay tuned.

Cushing’s triad is a late warning sign that a traumatic brain injury (TBI) has progressed to imminent brainstem herniation. Waiting for these terminal signs to appear before intervening in a Prolonged Field Care (PFC / PCC) scenario means you are already behind the curve.

When an intracranial bleed causes pressure to build inside the rigid walls of the skull, the body triggers three classic physiological responses to maintain cerebral perfusion:

• Widening Pulse Pressure
• Bradycardia
• Irregular Respirations

The Strategy: Treat Early and Aggressively

Evidence from the CRASH-3 trial and Joint Trauma System (JTS) Guidelines proves that your window of opportunity sits entirely in the mild-to-moderate TBI stage. If you wait for Cushing’s triad to manifest, the survival benefit of your interventions drops exponentially because structural failure is already underway.

To stay ahead of intracranial pressure (ICP), PFC protocols dictate immediate, proactive management:

• Aggressive Clot Stabilization: Administer Tranexamic Acid (TXA) as a single 2g slow IV/IO push within the strict 3-hour window from the initial injury. Head trauma triggers hyperfibrinolysis, causing the body to prematurely melt its own clots. As an antifibrinolytic, TXA stabilizes those early clots to halt hematoma expansion.

• Proactive Physiology Management: Optimize cerebral venous drainage immediately by elevating the casualty’s head to 30 degrees and keeping the neck in a neutral alignment. Monitor aggressively to prevent the “deadly dual insults” of secondary brain injury: hypoxemia and hypotension.

• Osmotic Intervention: If signs of herniation or pupillary changes develop during prolonged holding, immediately transition to osmotic therapy by administering 3% Hypertonic Saline to reduce brain swelling.

When evacuation timelines stretch into hours or days, passive observation allows intracranial pressure to take over. Treat the underlying bleed early, control the physiology aggressively, and protect the brain before irreversible damage sets in.

Post-Exercise Drills 🛠️📦

Another advanced tactical medicine course is officially in the books! Now comes the real work behind the scenes—the breakdown, inventory check, and deep clean. From heavy-duty hard cases and logistics bins to specialized trauma kit resets, we ensure every piece of gear is systematically organized, accounted for, and ready to deploy at a moment’s notice.

Training hard is only half the battle. Maintaining operational readiness is what guarantees success for the next rotation.

True readiness starts with structure. Moving through the stages of motor learning allows you to transform new movements into smooth, automatic application.

#LeaveNothingToChance
#ControlTheChaos

Step 1 is the Cognitive Phase. You are building a clean foundation by deliberately focusing on the perfect mechanics of a skill, like applying a tourniquet.

#Cognitive
#SkillDevelopment

Step 2 is the Associative Phase. Your rhythm improves as your movements become more fluid with less errors. This is where you comfortably build confidence in a controlled setting.

#Associative
#Training

Step 3 is the Autonomous Phase. The movement pattern is deeply established. Your hands execute the sequence smoothly on autopilot, allowing you to perform under any conditions with no errors.

#Autonomous
#Mastery

Step 4 is Tactical Application. Because your hands know exactly what to do, your mind is able to focus on your surroundings, situational awareness, and make proactive decisions.

Step 5 is Skill Maintenance. Keeping your skills sharp is easy with short, regular blocks of focused practice to protect your investment and keep your movements smooth.
