“You don’t need an iPACK.” Agree or disagree? 🤔

An ACB or femoral triangle block covers the front of the knee, but not the posterior capsule. What we see without an iPACK: more posterior knee pain for the first 3–5 hours after a TKA.

📲 Master every block, step by step, in the PNBschool Mobile app. DOWNLOAD IT FREE NOW 👉 tap the link in bio (App Store + Google Play)

FREE for everyone:
✅ Question of the Day pushed to your phone
✅ 16 regional block modules
✅ 3 pharmacology + 3 coexisting disease modules

Unlock with CME:
🔸 1,500+ board review questions
🔸 Over 70 pharmacology modules
🔸 60+ coexisting disease modules
🔸 Up to 10 hrs AMA PRA Category 1 Credit™ or ANCC contact hours

The evidence:
• Meta-analysis, 14 studies: ACB + iPACK vs ACB alone = lower pain scores, less morphine, no difference in quad strength (Guo et al., J Orthop Surg Res 2022)
• Without periarticular infiltration: less pain up to 24 h. With infiltration: no pain difference at 6 h (Hussain et al., Reg Anesth Pain Med 2021)
• 19 RCTs: no significant difference in pain scores or opioid use, but early function may improve (J Orthop 2025)

Our take: same prep as your ACB or femoral triangle block. It’s a simple field block of the posterior capsule. When the popliteal artery “lifts up” on the screen, that’s your sign of proper injection. Definitely worth it.

Bottom line: the evidence is mixed. Early posterior pain is real.

EtCO2 during CPR is one of the most misread numbers in a code. It reflects pulmonary blood flow, which means it is telling you how much cardiac output your compressions are actually generating. A value that stays under 10 mmHg after 20 minutes of high-quality CPR predicts a near-zero chance of ROSC, and a sudden jump above 35-40 mmHg usually means you just got circulation back.

That question is a real one from the PNBschool Mobile app board review system.

The PNBschool Mobile app includes:
• Board review system — 1500+ high-yield questions, timed exam and practice modes, and a misses log that shows you exactly where you are weak
• 16 regional block modules — anatomy, ultrasound, and technique
• Question of the Day pushed straight to your phone
• Over 70 pharmacology modules
• 60+ coexisting disease modules
• Up to 10 hours AMA PRA Category 1 Credit or ANCC contact hours

Most anesthesia board review courses run $400 to $900. The PNBschool board review system is $39.99.

🚫 STOP saying TKA patients don’t have posterior pain — they do.

The adductor canal block (ACB) or femoral triangle block are fantastic for anterior and medial knee coverage while sparing quadriceps strength — but it leaves the posterior capsule uncovered. That’s the exact region driving the deep, aching posterior knee pain so many patients report after total knee arthroplasty.

Enter the IPACK block (Infiltration between the Popliteal Artery and Capsule of the Knee). It’s fast, motor-sparing, and easy to layer in right alongside your ACB/femoral triangle block. By depositing local anesthetic in the plane between the popliteal artery and the posterior knee capsule, the IPACK targets the terminal sensory branches supplying the back of the knee — WITHOUT touching the tibial or common peroneal nerves. The result: posterior capsule coverage, preserved motor function, and a patient who can participate in early mobilization. Pair ACB + IPACK and you finally cover the whole knee.

Okay but can we talk about how being a CAA and being a professional referee require almost the exact same skill set?

Genesis Perrymond is both ✨ CAA and professional G LEAGUE referee.

In her PROCESS episode we talk about what it really takes to
• read the room in seconds
• make the call when everyone is watching
• regulate your own nervous system in high stakes moments
• stand in authority without being a bully

The overlap is wild.

You will also hear from Keisha, a practicing #certifiedanesthesiologistassistant who has worked locums with @harmonyanesthesiastaffing for the past five years and shares what that flexibility has meant for her life and career.

If you are expanding your idea of what this profession can look like, this one is for you.

In 2025 I primarily worked five eight-hour shifts a week (40 hours). I worked a few 24-hr weekend call shifts during the year but was usually off from work on weekends. I scheduled a couple of days off every month to spend time on travel, hobbies, and to rest. I found it to be a great *life*-work balance. I felt very present at work and I felt as though I had enough time outside of work to decompress or explore new hobbies.

This video is not meant to minimize that work days can be hard or draining. Being a CAA can be mentally exhausting because we have to execute at a high level day in and day out. Working a 8-12 hour shift as an anesthesia provider can also be physically taxing. We earn a great income but it comes with a high level of responsibilities and expectations. But as shift workers, we can take ample time off and achieve a balance of working hard while enjoying time off.

After investing aggressively over the last four years, I plan on continuing to tip the scale towards the life part of work-life balance. Others may prioritize working more. They may be working towards buying their first home, or be the sole financial provider in their family, or be paying off hefty student loans. A great part of being a CAA is that there are many work opportunities and for the most part you can scale your hours up or scale them down depending on your desired work life balance.
