@pazonamd calls it the lost decade. Ten-plus years of studying and training, and half a million dollars in debt at the end of it.

A lot of the loudest voices in wellness skipped all of that. They have huge followings and something to sell, and people listen. I’ve watched my own patients try whatever “protocol” went viral that month.

Then something real comes up. A lab comes back off, or a symptom won’t go away. Most of them come back to their doctor.

Patients are smarter than they get credit for. They can tell who’s selling something and who spent a decade+ learning how to take care of them. When they walk back in, all those years of training are the reason someone’s ready for them.

I would do it all over again.

(except organic chemistry, that sucked 😂.)

Because of my patients. 💙

What an incredible weekend at PepMed 2026.

I am so grateful to every one of you who showed up, asked thoughtful questions, supported this work, and continued to advocate for your own health. That willingness to stay curious and engaged is how medicine moves forward.

Peptide medicine is evolving quickly, which is exactly why conferences like this matter. The misinformation will be loud. So we keep showing up. We keep learning, researching, questioning, and teaching, because patients deserve conversations grounded in both possibility and responsibility.

I also feel incredibly lucky to stand alongside such an amazing group of physicians, researchers, and educators, including my longtime colleague @dr_abidhusain , who took the conversation beyond weight loss to explore the much larger cardiometabolic impact of incretin-based therapies.

Thank you, PepMed, for creating space for those conversations and for bringing this community together.

Now, straight to Austin for the Science of Skin Summit armed with my brand new @theonefrequency !

A really long week in DC ! And behind it, a battle most people will never see.

Six of the seven peptides reviewed by the FDA’s advisory committee were recommended for the 503A compounding list: BPC-157, TB-500, KPV, MOTS-c, Epitalon, and Semax.

I can’t help but feel emotional. This happened because of the relentless, behind-the-scenes work of so many incredible organizations - @internationalpeptidesociety, AAPM, and countless clinicians, pharmacists, lawyers, politicians, researchers and patients who refused to let this door close quietly.

To me, this is the first small step in making sure our patients have the right to work with their doctor to be in charge of their own health.

Make no mistake. There is still a long way to go for patient rights and freedom of choice in medicine. But I am deeply honored to have been part of this epic moment.

Peptides are making headlines again—and today’s FDA Pharmacy Compounding Advisory Committee (PCAC) meeting could have an important impact on the future of longevity medicine.

But what does that actually mean?

Yesterday, the PCAC voted to recommend that several peptides—including BPC-157, TB-500, KPV, and MOTS-c—be added back to the 503A Bulk Drug Substances list, which could allow qualified compounding pharmacies to continue preparing them if the FDA accepts the recommendation.

This is not FDA approval, and the FDA is not required to follow the committee’s recommendation. But it is an important step in the ongoing conversation about innovation, access, and evidence-based medicine.

As a physician, I believe the future of longevity isn’t about chasing trends—it’s about combining promising science with rigorous research and thoughtful patient care. Peptides may play an important role in that future, but our responsibility is to stay grounded in the evidence as it continues to evolve.

I’ll be following the FDA’s final decision today as well and sharing what it means for patients and the future of personalized medicine.

Another year around the sun.

We don’t grow by adding years. We grow by what the years ask of us — and just how we’re willing to answer.

Our bodies ask the hardest questions. For me, a brain injury in my twenties, competing and winning some of the hardest endurance events in the world, Long Covid decades later. Times when conventional medicine — my own training, my own profession — had no answers for me. What it did give me was curiosity: the refusal to accept “we don’t know” as the end of the sentence and the drive to keep looking — into the nervous system, into what the body is saying when tests say nothing.

That curiosity became BioHarmony®️ — a methodology born of my medical practice, my psychiatry training and a life spent learning what truly creates care, belonging and nervous system capacity. It kept evolving this year. From the Six Senses Ibiza RoseBar days to summit stages, workshops, the Mallorca retreats. The arc grew to honour what the nervous system actually is: dynamic, weather-like, never fixed. A safe container for emotional expression, growth, community and real connection.

And this year taught me again what I keep having to relearn: you cannot hack biology. You cannot force it. The very drives that make us feel switched on — the striving, the building, the always-more — are the same drives that, left unchecked, quietly write pathology into the body. The “on” feeling is seductive. Over time, it costs.

So yes, my life is full. But I carve out time for what refuels me — for self-care, for joy, for late nights dancing on the rocks in Ibiza. Because that isn’t indulgence. That’s the biology of joyspan: joy as physiology, not reward.

My daughter finishes primary school this month. A whole era closing as I write this. I’ve tried to be there for the small moments, not just the milestones. Some I caught. Some I missed. That’s the truth of a working mother.

Friendships deepened. And some business relationships I entered with grace and an open heart didn’t hold. It stung. It also taught me where my yes belongs.

The most overhyped thing in longevity? Peptides without context. 🧬

Loved this conversation with Dr. Jessica Shepherd — OB/GYN, author of Generation M, Chief Medical Officer at @hers, and Longevity Doctor of the year at @longevitydocsnetwork Cannes.

Her take: peptides aren’t bad, but “I’m just going to do peptides” without understanding why, how to stack them, or what your actual goal is? That’s the hype talking. Take the hype down, understand the purpose — then they can be useful.

And the most underhyped? The basics. Nutrition and exercise. The fundamentals are easy to do — and most of us still aren’t doing them enough. Get that Zone 2 in. 💪

Also — if you haven’t read her book on perimenopause and menopause, it’s genuinely excellent and a must for anyone navigating that transition. 📖

What do you think is most overhyped in longevity?

So honored to be Longevity Doctor of the Year!!

While in Cannes, I was surrounded by some of the most inspiring minds in longevity, and I am filled with so much gratitude and reflection. ✨

Being recognized as Longevity Doctor of the Year at the @longevitydocsnetwork Summit is an honor that means so much—not just because of the award, but because of the mission behind it.

Longevity, to me, has never been about simply adding years to life. It is about adding life that has purpose and quality…..especially for women in midlife.

It is an opportunity to understand our bodies, our hormones and our health in a completely new way.

Every day, I have the privilege of caring for my patients at @sanctummedwellness , building the Modern Meno @modernmeno community, and serving as Chief Medical Officer at @hers , where we continue to create innovative ways for women to access education, support, and personalized healthcare.

Longevity is not a template. It is a personalized, intentional practice that requires curiosity, proactive choices, and a commitment to designing the next 20–30 years of life with purpose.

Sooo…. How do you want to live?
