Internal cheek sculpting, or what Dr. Caughlin calls the “Cheekago”: tightening the buccinator muscle and mucosa on the inside of the cheek, sometimes with a little buccal fat pad sculpting, sometimes without.

Who it’s for: a round cheek, thick skin, a short ramus of the mandible, a fuller lower third that can take a bit of volume coming out.

Who he’s careful with: very thin skin, or a long ramus that’s already stretched.

That’s also why buccal fat removal alone can look sharp on one person and gaunt on another. Skin thickness, ramus height, the distance from jaw to cheekbone. The anatomy decides, not the procedure.
