I had a conversation with a doctor being told she’s “inefficient.” She doesn’t see patients “fast enough.” And despite working more than 80 hours a week, providing thorough, compassionate care, and charting into the evenings and working on weekends, she isn’t “good enough.” How can this be an ideal model for healthcare? Doctors and nurses need time with patients. We need time to be present. To provide thorough care. To check in with our staff. If you’re being told you’re inefficient, I see you. The friends that you make during the hardest periods in life stay in your heart forever. I met @neocookiemom during my first week of medical school and we were drawn to each other immediately. She’s Jamaican, I’m Guyanese and we were the only people from the Caribbean in our med school class. I remember: Walking to the hospital together in the dark, feeling like we were the only people alive at 5am, then racing each other to see who could finish rounding in the NICU first. Coming home hungry and exhausted on our surgery rotation, collapsing on the sofa and then sobbing because there was no food in the apartment. Only girls would cry about that! Doing an away rotation in Jamaica as fourth year students, and deciding that we’d stay out late to go to Carnival parties. Because $@@$ it, you’re only young once. Yet somehow we managed to show up bright eyed and bushy tailed to our rotations. Now we’ve both been in practice for over 20 years. Family doc and neonatologist. Mothers. Wives. Experienced. Dependable. But still always ready to have some fun together. Here’s the thing: years pass, we get busy and friends can slip away. So make time stay in touch. You don’t need to have many people in your life. You just need to cherish the ones who matter. Dedicated to my ride or die @neocookiemom. • Welcome to Lesson 29, Pre-Rounding, Part 2 Series: Preparation for the Wards • 🔹Welcome to Part 2 (yes, only part 2 because I had a lot of things to talk about in Parts 1 a/b/c! Which psst make sure to go watch!). Doing pre-rounding well will make rounds and overall patient care/safety much better. 🔹And ooh psst pre-rounding involves looking up each patient’s data & going to see the patient prior to rounds 🔹Going to see your patient during pre-rounds is more than just doing an exam. I recommend you do the following: ✅Talk to the patient’s nurse ✅Evaluate the room ✅PERTINENT exam ✅Know how to respond to patient/family questions 🔹Also, as I mention in the video, please ask your senior resident/attending if they want you to wake the patient up for the exam or if you can examine them on rounds. My rule of thumb is that if I think my exam will change management in any way or I’m worried about them, I wake them up. Otherwise I peek in +/- do a very quick silent exam (peds has made me an expert at the quick exam that doesn’t wake the patient) and save a fuller exam for rounds. ~ What questions do you have? Medicine is a team sport & there are so many fantastic teams that are also taking care of the patient! Here is a list of some of the most common/important notes you should glance at before rounds: ✅Attending Addendums ✅Consult Notes ✅Therapy Notes ✅Case Management & Social Work Notes ✅Op Notes ✅Nursing Notes ✅Chaplain Notes 🔹Chart-checking efficiently is a skill, just like presenting. • Welcome to Lesson 23, Pre-Rounding, Part 1b Series: Preparation for the Wards • 🔹This is only part 1b because I have A LOT of things to talk about 😁. Don’t forget to check out Part 1a! Doing pre-rounding well will make rounds and overall patient care/safety much better. 🔹And ooh psst pre-rounding involves looking up each patient’s data & going to see the patient prior to rounds 🔹Looking at the I/Os (intake & output) is often easily forgotten. This is crucial however to not overlook your patient’s nutritional and fluid status! Two big risks of being in a hospital bed for a while includes fluid overload & constipation, which can cause a lot of problems down the line if not addressed early! 🔹Looking at the MAR is also often overlooked, especially the PRN (as needed) med section. They were needing how many PRN pain or nausea meds? Why? Also, it’s important to make sure that your patients are getting the meds you ordered. Before you increase the lactulose for persistent HE (hepatic encephalopathy), make sure they haven’t been declining each dose. 🔹It’s also easy to overlook orders (see the theme?) but that is a must-view section. Anything ordered overnight that no one told you about? Anything you can de-clutter— do you really need daily labs? Q4h vital signs? That norepinephrine order still in despite pt being out of the ICU? Be nice to your nurses (and safe for your patients!) and leave the orders nice and neat. 🔹Chart-checking efficiently is a skill, just like presenting. • Welcome to Lesson 20, Pre-Rounding! Series: Preparation for the Wards • 🔹This is only part 1a because I have A LOT of things to talk about 😁. Doing pre-rounding well will make rounds and overall patient care/safety much better. 🔹And ooh psst pre-rounding involves looking up each patient’s data & going to see the patient prior to rounds 🔹It is an absolute MUST to look at the vitals first. I can’t tell you how many times I’ve gotten report from the night team and then looked at my patient’s chart to see that they are on max O2 support and no one told the night team or me. So look at the vitals!! 🔹Same with lab results. Now many hospitals have “critical lab alerts” which means that the lab usually has to contact either the patient’s RN or you to tell you about the lab value. But don’t rely on this! Now, some labs you can wait to discuss on rounds, like insulin management for a morning glucose of 250 for someone on nighttime glargine, but others (like an INR of 11!) you shouldn’t. 🔹It’s always good to check images even if you don’t remember ordering anything— sometimes the night team ordered something & forgot to tell you or perhaps you were waiting for the MRI read from the evening prior and hey now it’s back! 🔹Chart-checking efficiently is a skill, just like presenting. The more you do it, the faster you will get, so no fear if you take a while! Especially if the EMR (electronic medical record) is new to you! ~ What questions do you have? Was this helpful?
