No todo lo que encontramos en el fondo de ojos necesita tratamiento, pero sí puede requerir seguimiento.

Un nevus coroideo es una lesión pigmentada benigna.

Posterior scleritis

Today’s case was submitted by Addy Adkisson (@addyadkisson).

This 76YO female has been followed by us for several years with recurrent anterior scleritis. Her personal medical history on presentation was negative for established underlying rheumatic disease. She presented for a regular follow-up examination with symptoms of mild pain OD. Vision was 20/30 OD and 20/30 in her normal OS. Anterior segments were quiet.

Optos color RG imaging shows solid choroidal elevation involving the temporal peripheral retina and ciliary body (image 1). Choroidal folds are evident temporally on Optos as well as on Triton swept-source OCT (image 2). The choroid is markedly thickened with trace overlying macular subretinal fluid. Fluorescein angiography shows speckled hyperfluorescence overlying the areas of peripheral choroidal elevation, more diffuse leakage along its posterior extent, late disc staining, and ill-defined submacular leakage (image 3). B-scan ultrasonography (image 4) shows a diffusely thickened choroid with hyporeflective fluid posterior to the sclera (T-sign).

Posterior scleritis can present either as a diffuse thickening of the entire posterior sclera or as a nodular type with localized scleral thickening (Agrawal et al, Retina 2016;36:392-401). Nodular inflammation appears as a posterior pole subretinal yellowish mass lesion, often with overlying serous fluid and chorioretinal folds. B-scan ultrasonography may show fluid in the Tenon space (“T” sign). Unlike anterior scleritis, the eyes are usually quiet and pain-free. Initial treatment is usually with oral non-steroidal anti-inflammatories or corticosteroids, although patients may also require immunosuppressive therapy. For a large case series of posterior scleritis, see McCluskey et al, Ophthalmology 1999;106:2380-2386.

High myopia with macular hole posterior pole rhegmatogenous retinal detachment

This 39YO female has been followed by us for over 20 years with high myopia and lattice degeneration. At the last examination 6 months earlier, vision was 20/70 OD. She returned 6 months later with asymptomatic decreased vision in her right eye. Vision was counting fingers.

Optos color RG imaging shows a rhegmatogenous retinal detachment (RRD) extending from the macula into the midperiphery (image 1). Triton swept-source OCT confirms the presence of the causative macular hole. A partial PVD temporally extends and elevates the nasal edge of the hole, with complex elevated epiretinal membranes extending towards the nerve. The detached neurosensory retina is diffusely thickened with mostly outer nuclear layer cysts. The hole and detachment are best visualized with the green channel (image 2).

RRD with concurrent non-causal macular holes are uncommon, occurring in 2% of eyes (Iros et al, Retina 2023;43:581-584). Repair of the causative peripheral break(s) without addressing the MH nearly always repairs the detachment and the MH, although many will also perform internal limiting membrane peeling. We have seen several high myopes, including our current patient, with macular holes causing posterior RRD. Vitrectomy in these eyes also has a high surgical success rate, although persistent subretinal fluid may be more common (Cheng et al, Retina 2024;44:782-790).

Three months following vitrectomy and subsequent cataract surgery, vision improved to 20/200. Although the retina was completely attached (image 3), the macular hole remained open clinically (arrow) and on OCT. Our patient was functioning well and deferred further surgery.

Dengue retinitis
Mosquito: 1, Retina: 0

Today’s Retina Foundation case of the week was submitted by Rohan Jain (@rohan_jn93) and Manish Nagpal (@drmanishnagpal).

This 63YO male presented with 1 month of bilateral decreased vision following dengue fever, with positive Dengue NS1 antigen during the acute illness. Vision was 20/200 OD and counting fingers OS.

Pseudocolor SLO imaging shows extensive peripapillary cotton wool spots, retinal whitening, and significant macular edema (image 1). The fundus detail is partially obscured by overlying vitreous debris from vitritis. OCT scanning neurosensory retinal detachment, intraretinal cystoid edema, and subretinal fluid involving the macula in both eyes, consistent with post-dengue inflammatory retinitis.

Topical steroids and a 9-week course of tapering oral steroids, as well as a 3-week course of oral doxycycline, were started. Four months later, vision improved to 20/20 OD and 20/30 OS. The patches of white inner retinal ischemia are fading, and some lipid flecks have precipitated in the mid-retinal region (green box and arrow).

Epidemic (post-fever) retinitis is an infectious or para-infectious entity that develops in immunocompetent individuals 2-6 weeks following a bacterial, viral, or protozoal infection. Rickettsiosis is the most commonly identified cause, though a large proportion of cases remain idiopathic despite serologic testing; dengue, chikungunya, West Nile virus, Zika virus, leptospirosis, typhoid, and Ebola have also been implicated. Patients present with sudden painless vision loss. Fundus findings include unilateral or bilateral unifocal or multifocal patches of retinitis with possible nerve involvement, neuroretinitis with macular star, serous macular detachment, cystoid macular edema, and vasculitis or frosted branch angiitis.
