Anterior Drawer Test: Mastering ACL Stability** Struggling to remember the exact angles for knee special tests?

Let’s break down the Anterior Drawer Test a staple in clinical examination for assessing the integrity of the Anterior Cruciate Ligament (ACL).

The Setup (Precision is Key!)
Patient Position: Supine with the hip flexed at 45°and the knee flexed at 90°

*Stabilization:Sit on the patient’s foot (avoiding excessive weight) to prevent the lower limb from sliding.

The Execution
Grasp the upper tibia with both hands, ensuring your thumbs rest on the joint line.

Apply a firm forward pull (anterior translation) of the tibia.

Interpreting Results
Positive (+) Result:A significant forward shift of the tibia or a "soft/mushy" end-feel suggests ACL instability (Sensitivity: 62%, Specificity: 88%).

Posterior Drawer Sign: Always watch for a "sag" before you pull! If the tibia is already shifted backward, it might indicate a PCL injury instead.

Pro Tip:Never assess in isolation! Always compare with the contralateral (uninjured) knee to establish the patient’s normal baseline laxity.

Save this post for your next MSK clinical rotation!
