I attended the 2026 Patient Day organised by Ziwig For Health, hosted at the Ziwig Lab, the laboratory that develops the Ziwig Endotest®, a saliva-based diagnostic test for endometriosis.

I was there as an endometriosis advocate and speaker, with two goals. First, to speak during the Agora and carry the voice of patients. Second, to visit the laboratory and see, concretely, the journey of a Ziwig Endotest® from saliva sample to result.

In the lab, we followed the journey of an Endotest®, from the arrival of the saliva sample to the result. It takes on average 10 days, with 3 key steps:
1) Extraction: the microRNAs are isolated from the saliva
2) Sequencing: they are read using next-generation sequencing (NGS)
3) Analysis: artificial intelligence analyses a signature of 109 microRNAs

Within its current framework, it is meant for people aged 18 to 43 whose pain suggests endometriosis but whose imaging is negative or inconclusive, who are not pregnant, with no history of cancer and who are not HIV-positive.

Today, I see the Endotest®’s place in the care pathway clearly. For people within its current framework, it can help avoid a diagnostic laparoscopy or guide further imaging, for example prompting a closer review of a previous MRI or a more targeted new one. Fertility isn’t an indication in itself. But for people tested for pain who are also trying to conceive, considering endometriosis early can save precious time.

In its validation study published in NEJM Evidence, involving 971 symptomatic women across 17 hospitals in France, the test correctly identified around 97 out of 100 women who had endometriosis, and correctly ruled it out in around 94 out of 100 women who did not. Overall, it gave the right result in about 97 out of 100 cases.

I am looking forward to the next steps in research. In France, the Ziwig Endotest® is currently covered on a temporary basis through the “Forfait Innovation”, alongside a clinical study whose results will be decisive in defining its long-term place in the endometriosis care pathway.

Transparency note: I joined this event as part of a speaking engagement with Ziwig For Health. The views shared here are my own.
