🦵CHRONIC RIGHT KNEE PAIN | ASSESSMENT → LOADING

A 4-year history of right knee pain following a hyperextension injury, with no formal diagnosis or imaging at the time.

The athlete presented with:
• Poor quadriceps recruitment
• Pain + swelling immediately and the day after basketball
• Anterolateral and posterior knee pain
• Reduced plyometric force production and absorption

🔬 OBJECTIVE TESTING
VALD testing identified:
📉 CMJ: Reduced jump height + eccentric peak force
📉 SL CMJ: Reduced jump height on the right
📉 Calf strength: ~10% right-sided deficit
📉 SL knee extension: ~12.5% right-sided deficit
📉 Hip abduction: Reduced strength/recruitment bilaterally

🦵 MOVEMENT STRATEGY
Jump analysis demonstrated:
• Reduced eccentric knee flexion
• Reduced concentric plantarflexion
• Increased right hip flexion to achieve foot clearance and promote flight time

The athlete appears to be redistributing force production away from the knee and ankle, with increased hip contribution in the presence of reduced eccentric knee flexion and concentric plantarflexion.

Combining the objective data with clinical and movement assessment provides a clearer picture of the athlete’s force production, force absorption and load distribution.

🏋️ REHAB PRIORITIES
1. Heavy SL knee extension
Increase quadriceps strength and capacity while progressively loading the knee. Bridging the asymmetry gap 🔑

2. Hip abduction loading
Address bilateral hip strength and recruitment deficits.

3. SL calf loading
Restore plantarflexor strength and force production, also bridging the asymmetry gap.

4. Progressive plyometrics
Reintroduce eccentric loading, force absorption and concentric force production before progressing toward basketball-specific demands.

The objective isn’t simply to strengthen the knee - it’s to restore the capacity of the entire lower limb to produce, absorb and transfer force under increasing sporting demands 🏀⛹🏽‍♂️
