Everyone’s talking about peptides. Almost no one’s talking about the other half of the equation. 🧬

Here’s what surprised me even after years in medicine: about 1 in 3 of your body’s own peptide hormones come pre-attached to sugar chains called glycans — and that sugar is what keeps them from being shredded in minutes.

Three you already know:
🩸 EPO — ~40% sugar. The blood hormone athletes get banned for.
🤰 hCG — ~30% sugar. The pregnancy hormone home tests detect.
🦋 TSH — your thyroid’s master switch, same sugar signature.

Strip the sugar off any of them and the hormone falls apart — cleared in minutes, signal lost.

Glycans aren’t just decoration on hormones, either. They form the glycocalyx — the fuzzy coat on every cell, including the lining of every blood vessel you have. When that vascular coat thins out (high blood sugar, inflammation, smoking), it’s one of the earliest signs of cardiovascular trouble.

So here’s how I think about it:
👉 Peptides are the message.
👉 Glycans are the language — and the protection around it.
👉 Your gut, vessels, and immune system are the terrain it all runs on.

The next frontier in longevity isn’t a single molecule. It’s supporting the whole communication system at once. Nature never optimized one and ignore the other — and neither should we.

Ten cases today. Morbidly obese patients. Serious structural problems. Cases other surgeons turned down.

And I need to say something that might make some of my colleagues uncomfortable.

Stop saying no to patients who need you.

I get it. Morbidly obese spinal surgery is hard. The risk profile is higher. The exposure is more complex. The anesthesia is more demanding. Every variable is more challenging.
I am not dismissing that.

But there is a difference between a surgeon who says “let’s optimize you medically before we operate” — which is the right call for elective cases where timing allows — and a surgeon who looks at a patient with progressive disability or even deficits, a closing window for intervention, and real permanent damage accumulating in real time —
And says no because the case makes them uncomfortable.

That second surgeon is failing their patient.

And here is the part that actually keeps me up at night:
When enough surgeons make that decision — the skill to safely do these cases disappears.

You do not maintain surgical capability by avoiding difficult patients. You build it by showing up for them. Consistently. Over years. Through the cases that test your training and push your technique and make you better at the cases that come after.

I finished case number ten an hour ago.

I am already thinking about tomorrow.

Because the patients sitting at their seventh surgical opinion did not run out of options.

They just ran out of surgeons willing to show up.

Tag a patient who deserved a yes and got a no. This needs to be said. 👇

A Beverly Hills living room. One of the most powerful women in the world. And a conversation that is about to change how you think about every room you have ever walked into.

@mariediamondofficial — Feng Shui grandmaster, featured teacher in The Secret, advisor to Grammy winners, Oscar recipients, and some of the most successful people alive — just sat down with me for Wellness at the Speed of Light and said something in the first five minutes that I have not stopped thinking about since.

“The room you are sitting in right now is either giving you energy or stealing it. There is no neutral.”

No neutral.

Think about that for a second.

The desk you work at. The bed you sleep in. The direction you face when you make your most important decisions. Marie has spent decades proving — with thousands of transformed lives as her evidence — that these are not aesthetic choices. They are energetic ones. And the difference between a space that supports your health, your creativity, and your vitality versus one that quietly depletes it is something most people spend their entire lives never understanding.

Feng Shui is not what you think it is.

It is not candles and crystals and moving your couch three inches to the left.

It is the 3,000-year-old science of understanding that you are in constant energetic relationship with the space around you — and that relationship is either working for you or against you.

I went to Beverly Hills to understand it firsthand. What I came back with changed how I look at every space I walk into.
This is just the teaser.

The full episode of Wellness at the Speed of Light is coming and I promise — you have never heard a conversation like this one.

The “Ortho vs. Neuro” spine debate is a trap. Yes in some conditions it may make sense but generally, this is asking the wrong questions!

As a spine surgeon and President/CEO of a combined Ortho-Neuro practice, I’ve hired, managed and work daily with both. I see the raw data the public doesn’t.

The medical literature is clear: analyzing tens of thousands of surgeries reveals ZERO statistical difference in outcomes between the two titles. If you pick a surgeon based on the letters on their door, you are gambling with your spine.

Stop looking at their department and start auditing these 3 metrics:

1️⃣ Fellowship Training (highly specialized)
Did they complete a dedicated spine fellowship? Those that put in the sweat and effort to go through these intense training programs are more equipped to handle complex spine surgery cases and this is really not something in question.

2️⃣ The Reps (Specific Case Volume)
Don’t ask, “How many surgeries do you do?” Ask, “How many times have you done this exact procedure in the last 12 months?” High volume equals shorter OR times and fewer complications. Reps dictate mastery.

3️⃣ The Whisper Network (Actual Reputation)
Online reviews are a baseline, but the real cheat code is asking the hospital staff. Ask the OR nurses, scrub techs, and local physical therapists who they would send their own mother to. Staff see what happens behind closed doors when things go wrong. Their unvarnished opinion is the ultimate metric.

Making the difficult decision on who is going to take care of your spinal condition should be based on these three metrics rather than on whether a doctor is a neurosurgeon or an orthopedic spine surgeon.

Let me know if you’ve had experience with one of these specialties or both treating you for your spine condition and what your experience was and your thoughts on this debate. I’ll answer each and every one of the questions myself.
