Dentistry has increasingly moved toward extraction and implant replacement for teeth with a guarded long-term prognosis. Implants are an excellent treatment option, but we have to be careful not to let our own biases overshadow patient autonomy.

I often hear phrases like “confuse them and you lose them” or that patients should simply be told what they need. I don’t subscribe to that approach. If I can’t help a patient make an informed decision about their own health, then the problem is my communication - not the complexity of the decision.

My goal is to present the options honestly, discuss the risks and prognosis of each, and answer whatever questions the patient needs answered. Sometimes the most predictable treatment is extraction and implant placement. Other times, a patient understands the uncertainty and still prefers to try to save a natural tooth.

This patient had already undergone extensive grafting to prepare for an implant on the opposite side and wanted to avoid additional surgery at this time. After discussing the prognosis, risks, benefits, and alternatives, she elected to save the tooth.

Will it last forever? Maybe not. It may fail in a few years. But I’ve also seen teeth with questionable prognoses last decades when restored thoughtfully and maintained well. And if it does fail in the future, implant therapy remains an option.

Careful treatment planning, precise execution, biologically friendly materials, and well-designed restorative contours have given this tooth another chance. Time will tell the rest of the story.

It’s All Coming Back to Me Now played softly while a Kings of Leon drummer got his grill photographed.

I can’t remember who hit play on @celinedion … We just all decided to commit.

@spaceofdonder at @nashville_aesthetic_dentistry designed it.

@styledentlab built it.

@thenathanfollowill of @kingsofleon wore it like he was born with it.

My job was to document it with the seriousness it deserved. Celine and all.

Mission accomplished.

When an implant isn’t the right fit, thoughtful planning makes all the difference.

After extraction due to periodontal bone loss, we restored this space with a conservative bridge. By utilizing the adjacent teeth that had existing crowns for support, we were able to avoid unnecessary treatment and preparation of teeth without an existing restoration. Shaping the tissue with a provisional restoration at the time of tooth extraction allowed the tooth to emerge naturally from the gums.

The result: a restoration that’s esthetic, hygienic, and designed to last.
