For an anteroposterior (AP) X-ray of the tibia, place the patient supine or sitting, fully extending the leg with the patella facing straight up. Dorsiflex the foot so toes point to the ceiling. Center the beam on the mid-shaft to include both the knee and ankle joints. [1, 2, 3]
Patient Preparation and Setup
Position: Patient is supine on the table or sitting upright with the affected leg fully extended.
Limb Alignment: Ensure the leg has no rotation; the patella must face directly toward the ceiling.
Foot Position: Dorsiflex the foot so that the bottom is near perpendicular and toes point up (use a supporting band if needed to hold the position). [1, 2, 3]
Exposure and Beam Direction
Central Ray: Directed perpendicular to the midpoint of the tibia/fibula shaft.
SID (Source-to-Image Distance): Standard 100 cm (40 inches).
Joint Coverage: Collimate to include both the proximal joint (knee) and distal joint (ankle) on the image receptor. If the cassette is too small for very tall patients, specialized separate views of the joints may be required. [1, 2, 3]
If you need guidance on a different view, let me know if you would like the lateral position or instructions for lower extremity trauma protocols.](https://s2.imginn.com/782362387_18091059749181590_6647415739253973245_n.jpg?t51.82787-15/782362387_18091059749181590_6647415739253973245_n.jpg?stp=dst-jpg_e15_p480x480_tt6&_nc_cat=102&ig_cache_key=Mzk2OTA5ODE4OTQ0MTM5MzY0NTE4MDkxMDU5NzQzMTgxNTkw.3-ccb7-5&ccb=7-5&_nc_sid=58cdad&efg=eyJ2ZW5jb2RlX3RhZyI6IkNMSVBTLnhwaWRzLjU0MC5zZHIudmlkZW9fZGVmYXVsdF9jb3Zlcl9mcmFtZS5DMyJ9&_nc_ohc=fbTuFdIFgJkQ7kNvwEAku6T&_nc_oc=AdpxeO-svFx9Nbm7HdXFMxNjLZdleXVBycWcHh_cIagS7AK0tr8anj8TcmibfrMDpGZqi-h_zClunzMUaOt_h8GQ&_nc_ad=z-m&_nc_cid=0&_nc_zt=23&_nc_ht=scontent-atl3-2.cdninstagram.com&_nc_gid=4kxL1_2zCIDhkcw3PJmYdg&_nc_ss=7a20f&oh=00_AQOXttq7XlQSdntCE1LuLb-Q_Kb0KsHt34iVv5NvzS20Sg&oe=6ADA2ED9)
