👋🏻 A quick reintroduction for those who don’t know me!

🌍 I’m Jenni, a chartered physiotherapist with 11 years experience across Ireland, New Zealand and the UK. 

🦴 I began working in bone health following my mums osteoporosis diagnosis and have never looked back 🦴 I started working with her to improve her bone health through exercise.

🩻Seeing how many people were affected by low bone density and how little bone health services existed I began setting up bone health programs in physio practices in Ireland and the UK. 

🏋🏻‍♀️ These programs aimed to educate people, and build strength and balance, helping to improve bone health, reduce falls and fracture risk, decrease fracture related pain and empower people to live their life confidently.

🦴 I've worked with people who have osteoporosis, osteopenia, RED-S, vertebral fractures, stress fractures, rheumatology conditions and other conditions that affect bone health.

💕 I’m passionate about providing evidence-based education and empowering people to exercise, improve their bone health and outcomes and live healthier and stronger for longer!

👩🏼‍🤝‍👨🏽 1 in 2 women, and 1 in 5 men, over 50, will break a bone due to poor bone health - it’s a huge growing problem and it needs to be addressed.

I’ve had a number of enquiries recently about booking a physio session or consultation with me so I have opened some time in my diary over the next 3 months. 

I am currently opening online appointments, this can be for physio, a one off consultation or to review your current program 🦴 

These are open to anyone - not just if you have osteoporosis 🦴 While most of my caseload and work focuses on low bone density I have a number of clients I am working with who do not see me for their bone health.

I began specialising in osteoporosis and bone health was when my mum was diagnosed with osteoporosis in 2019. 🦴 

The above slides give an overview of mums osteoporosis journey and how she has not only improved her DXA T-score but is more active, confident and pain free. 👏

When she was diagnosed, despite being a nurse mum found it very difficult to find trusted information and support that was practical for her. 

Mum has spent years being consistent with exercise, nutrition and lifestyle factors trying to reduce her risk factors and improve her overall bone health 🦴

While she is delighted that her has improved on her DXA scan - her big takeaway is how strong and confident she feels with exercise and activity. Prior to her diagnosis she struggled with constant neck and shoulder pain - yet today she no longer experiences ongoing pain despite being more active 🏃🏻‍♀️🏌🏻‍♀️

Mum is a huge advocate for bone health and anyone who knows her will have surely been asked if they know their risk factors or if they have had a DXA scan! 😂🦴

The most important thing you can do to improve your bone health is to be consistent 🦴 

Consistency is important with exercise, nutrition, lifestyle changes or even medication. 🏋🏻‍♀️🥑🚭💊

If you are trying to improve or maintain bone mineral density or slow bone loss through exercise it is important that you find a way to build exercise into your life. 

Changes to bone mineral density often take more than 12 months and we need to maintain the exercise to keep those gains 🦴

We will all get busy and skip workouts here and there (including me!) but it’s important to stay as consistent as possible - even if that means just doing a quick 10 - 20 minute workout when you can 💪

Just because you don’t have access to your gym or usual weights doesn’t mean you can’t improvise and do a routine that still covers all basics - even when travelling🦴 

This quick 20 minute workout done on a balcony in Sicily includes most of the key movements you need in your bone health program: 
⚪️ Pull 
⚪️ Hinge 
⚪️ Squat 
⚪️ Press 
⚪️ Impact

Insights on exercise and osteoporosis from the osteoporosis conference 🏋️🦴

The conference featured some incredible researchers and specialists presenting on the role of exercise and physical activity in osteoporosis and fracture rehabilitation (Professor Katherine Brook-Wavell,
Professor Karen Barker, Dr Julie Whitney)

There were no new research break throughs, or updates or changes to guidelines presented - but important to cover the evidence base that we have and what people should be prescribing as part of osteoporosis prevention and management. 

Over the 2 days there were 3 lectures on exercise 
🦴 Rehabilitation and activity in osteoporosis: A practical guide 
🦴 Exercise and mechanical loading on bone 
🦴 Exercise and physiotherapy after vertebral fracture: A practical guide 

While some lectures weren’t focused solely on exercise they also highlighted the importance of exercise in certain scenarios, like during weight loss or pelvic fracture recovery 🏋🏾‍♂️

Exercise is one of the few interventions that can both increase bone strength and reduce the risk of falls and it is important that we prescribe it appropriately to people to help improve their outcomes 🦴

As with the last two posts this are very brief 1 sentence highlights from full lectures - so there will be lots more nuance to each point!

Today is world physio day so it’s only right to address the role of physiotherapy in osteoporosis prevention and management 🦴

Physiotherapy plays a role across the whole osteoporosis pathway - from prevention to fracture recovery. 

There are many ways physios can help improve bone health outcomes: 

🦴 Prevention - prescribing the right exercise to help build and maintain bone strength
🔍 Screening - noticing risk factors or early identification of vertebral fractures 
⚖️ Falls prevention & balance - running progressive, tailored programmes that reduce fall risk
🏥 Fracture rehab - supporting early mobilisation and safe recovery after a fracture
🏃🏼‍♀️ Exercise - prescribing an effective, suitable exercise program to help you achieve your goals and positively impact your bone health 
🤝 Referral - connecting people with GPs, appropriate services and community falls programmes

You are never too young or too old to improve your bone health 🦴 If you are concerned about your bone health, have had a fracture or are worried about moving safely, a physio can help you find a safe, confident way back to activity.

Did you know that coeliac disease can affect your bones?🦴

When gluten damages the small intestine, it can reduce how well your body absorbs calcium and vitamin D, and bone loss is a recognised complication. 

Studies have shown that low bone mineral density is common at diagnosis, with low bone density present in 38% - 72% of adults at the time of diagnosis 🦴

The good news is that strict adherence to a gluten-free diet has been shown to improve bone mineral density 👏

Guidelines recommend: 
⚪️ DXA scanning and monitoring at the right points
⚪️ Lifestyle interventions (exercise, nutrition, addressing risk factors) to support recovery.
⚪️ Referral to specialist care in certain circumstances

Being diagnosed with low bone density, breaking a bone or being told you are at an increased fracture risk can feel very overwhelming. 

Many people are concerned about what activities they can do or if they should be doing them. 🤷‍♀️

As a result I see people reduce their activity levels and stop certain activities to try and prevent injuries. 

But being told you have osteoporosis shouldn’t be a reason to reduce your activity - instead it’s a reason to build strength and balance and remain as active as possible 🦴 

If you are concerned about what activity you can do reach out to a physiotherapist, exercise specialist or your medical team who can help guide you and keep you doing the things you love 💃🏼

How much balance training do you need to reduce the rate of falls? ⚖️

A major review pooling 88 trials and nearly 20,000 older adults (Sherrington et al, 2017) found that exercise reduced fall rates by 21%. 

But when programs combined challenging balance exercises (reducing your base of support, shifting your centre of mass, minimal hand support) with at least 3 hours of exercise per week, falls dropped by up to 39%.

Balance training works, but it has to be challenging enough and done at a high enough weekly volume to see this level of falls reduction.

Challenging will mean different things to different people. For some people it might be some of the lower level exercises I posted earlier in the week and for others it might be the exercises in this video.

As part of the weekly exercise its important to include strength or resistance training too, particularly functional strength exercises that mimic everyday movements: 
🏋🏻‍♀️ Squats or Sit to Stands 
🏋🏽‍♂️ RDLs or picking things up from the ground 
🏋🏼 Step-ups or stair walking 

Here are some standing high level or difficult exercises that cover key types of balance training: 
🧍‍♀️ Static: control while staying still 
🧠 Anticipatory: prepare before you move
🚶‍♀️ Dynamic: stay steady while moving
↔️ Functional Stability: explore your limits safely
⚡ Reactive: recover when you lose balance

⚠️ If you have poor balance make sure you are beside a wall or have support so you do not fall.
⚠️ This may not be suitable for everyone, if you are unsure do not try this at home and ask your healthcare professional next time you see them.
