She said her confidence was a 0 before starting.

After working through real patient scenarios and structured clinical thinking…

She said she’s now at a 3.5 out of 5.

Not perfect.
But no longer second-guessing herself.

That’s the shift.

Your assessment starts before you touch the patient.

Before you get focused on the medication, dressing change, or task that brought you into the room—notice.

What does the patient look like?

How are they breathing?

Has anything changed?

What are the monitor, lines, equipment, and environment telling you?

Clinical judgment starts with learning to notice what matters.

And that develops with practice. 💙

Experienced nurses: what’s the first thing you notice when you enter a patient’s room?

The next Transition Room cohort is coming. 💙

The Transition Room was created for the space between learning nursing knowledge and developing greater experience applying that knowledge in practice.

Our educational focus includes:

• Clinical judgment
• Prioritization
• Communication
• Practice through simulation
• Reflection and support

This isn’t nursing school part two, and it isn’t intended to replace employer orientation.

It’s an additional learning environment designed around opportunities to practice thinking through clinical situations, receive feedback, reflect, and continue developing.

If you’re approaching the transition from nursing school into professional practice—or you’re already navigating that transition, you can learn more about what’s ahead through the link in our bio.

I didn’t create The Transition Room because I believe nursing students and new nurses simply need more information.

There’s already a lot to learn.

What kept standing out to me was the space between:

“I know this.”

and

“I can use what I know in the situation in front of me.”

Prioritizing.

Making decisions.

Communicating concerns.

Working through uncertainty.

Receiving feedback.

Reflecting and trying again.

That’s the space I wanted The Transition Room to help nurses practice in.

💙 Tomorrow I’ll introduce the next Transition Room cohort and who it’s designed to support.

Nursing students and new nurses, I want you to hear this from someone who’s been in nursing for more than 20 years:

You are not going to know everything.

Neither do I.

Nursing will continue to give you situations you haven’t encountered before, questions you need to investigate, and moments when another nurse’s perspective matters.

Experience doesn’t mean never needing help.

It changes the patterns you recognize, the questions you ask, and how you respond when you don’t immediately know.

Give yourself permission to be developing while you’re learning. 💙

Experienced nurses: what’s something you understand now that you wish you understood during your first year?

Nursing students and new nurses—clinical judgment isn’t just about whether you know the answer.

It’s also the thinking that helps you decide what to do with the information in front of you.

NOTICE: What am I seeing?
INTERPRET: What might it mean in context?
PRIORITIZE: What needs attention first?
RESPOND: What should happen next?
REFLECT: What can I learn from what happened?

Real patient care won’t always fit neatly into five steps, and this isn’t a substitute for clinical assessment, organizational policy, supervision, or appropriate escalation.

But having language for your thinking can help you examine and strengthen it.

💙 Save this and come back to it during clinical or your next shift.

Nursing students and new nurses—you can understand something in class and still hesitate when you’re asked to apply it in a clinical situation.
That’s one reason practice matters.

Simulation can create an opportunity to:

Make a decision.
Explain your thinking.
Receive feedback.
Reflect.
Try again.
The goal isn’t perfection.
It’s an opportunity to practice using what you know.

💙Nursing students + new nurses: what situation do you wish you could practice before encountering it for real?

DAISY nominee 🤍
Certificate of Nomination, Pennsylvania State Nurse Leader DAISY Award, @ponlnetwork Annual Conference.
Over two decades a nurse. My staff believed in me before anyone else did, and on Friday Lily and Gloria stood beside me.

To every new nurse finding her way: keep going. Your moment comes.

“Something feels off” can be an important signal, but it isn’t where your clinical thinking stops.

When something about your patient doesn’t seem right, give that feeling somewhere to go:

Reassess. Compare. Trend. Communicate.

Reassess what you’re seeing now.
Compare it with the patient’s baseline and previous findings.
Look for trends and changes over time.
Communicate what changed and what concerns you.

Clinical judgment isn’t about having every answer immediately. It’s about knowing how to move from “something feels off” to a more focused assessment and appropriate next step.

Save this clinical-thinking sequence:
reassess → compare → trend → communicate

Nursing students and new nurses: save this one.

When your brain is telling you everything needs to happen right now, give yourself a process.

Start with:

What changed?
What presents the greatest immediate risk?
Who needs reassessment?
What cannot safely wait?
What can wait?

This isn’t a substitute for assessment, clinical judgment, your organization’s policies, or asking for help when you need it.

It’s a way to begin organizing your thinking when everything feels important at once.

Prioritization develops through practice, feedback, reflection and experience. 💙

Which of these questions do you want to remember on your next shift?

Sometimes clinical judgment starts with:

“Something doesn’t look right.”

You may not have the answer yet.

Stop. Reassess. Compare. Look at the trend.

Then ask yourself:

What is different from before?

New nurses don’t need to have every answer before recognizing that something has changed.

The next step is turning what you’re noticing into information you can assess and communicate.

💙 Save this for the shift when you start second-guessing what you’re noticing.

New nurses: one of the hardest parts of the transition isn’t having things to do.

It’s deciding what needs your attention first.

When everything feels urgent, start here:

What changed?
What presents the greatest immediate risk?
Who needs to be reassessed?
What cannot safely wait?

Prioritization isn’t about moving faster.
It’s learning to recognize what matters now.

💙 Experienced nurses: what helped prioritization finally start to click for you?
