Who are Vaginoplasties for? Is it for only women who’ve had vaginal childbirth? No. There are many many cases where a pregnant mom pushes for hours and hours, but just can’t deliver her child. Or there is a sudden fetal distress needing a C-section. The baby has damaged the mom’s pelvis and all tissues have been stretched, but delivery was by C-section. These are the women that can sometimes have significant damage and vaginal laxity and can be considered for a Vaginoplasty or even a Perineoplasty.

What is Cliteropexy Surgery? It is the suspension of a clitoris that bulges out, is “too big,” pokes out like a mini penis. It is the clitoris placed closer to the pubic bone, deeper under the clitoral hood. Reduce the visible appearance of that clitoral bulb sticking out and secure it safely under the clitoral hood. Done in the office under local anesthesia. About an hour of total time. Life changing and confidence inspiring. Specialist here in Arlington, Texas!

Most of the women who fly to Arlington are not coming for a first operation.

They are coming because a prior labiaplasty or vaginoplasty left pain, asymmetry, or too little tissue. Some come for FGM reversal. Revision is a different operation from a primary. The plan starts with what is left, then 3D thinking and planning, then the creation of art.

Feathering and Grooving are how I repair tissue that was cut too short, or cut away.

Feathering blends an abrupt edge. Grooving rebuilds a fold when the minora is gone. I use both in revision labiaplasty and in FGM reversal. The goal is a natural edge. Labia from no labia. Invented in Laguna Beach and taught to my Fellows since 2006. Grooving is now at home here in Arlington, Texas.

Hormone care is part of the Arlington visit when the history calls for it.

I have run a bioidentical practice for about 30 years. In consult we cover menopausal symptoms, local estrogen, and whether a compounded cream or thyroid support belongs in the plan. Peptides are a separate discussion, not a default. Now Amy Haddad, RNP, helps coordinate management and care. What an amazing team!

Clitoral hood reduction is its own operation. It’s not taught in Residencies or Fellowships.

On Gynflix I separate central hood reduction from lateral and medial hood reduction, and I say when I do one or none. The hood is not extra labia. It is its own structure. The block, the marks, where to begin and where to stop endpoint are the teaching.

I have taught labiaplasty and vaginoplasty repair techniques worldwide for more than 20 years.

Gynflix is where surgeons see the named moves — Barbie, Hybrid, Rim, Feathering, Grooving — and the revision pathways when a first surgery failed the patient. Teaching repair is not the same as teaching a first case. Tissue is already spent. Scars redirect the plan.

“Botched” is a word patients use. I use plainer language in the room. I had to develop special techniques and tools and instruments to fix problems deep inside the pelvis. The one I am holding is called the Lone Star APS Vaginal Retractor. It allowed me to see the problems.

Over-resection. Notch or dog-ear. Uneven Barbie or Hybrid lines. Rim that was never a Rim. Hood left bulky while the edges are tight. Vaginoplasty that feels tight at rest and fails with activity. Perineum that looks closed on the outside and still gaps under load. Closed look on the outside and loose and spongy on the inside. Sometimes the problem is deeper scarring and nerve entrapment. I examined carefully to find the problem.

Most of the women who see me for revision did not start here. They heard about me from my international teaching and local doctors. They know I have won awards for my work. So they come to Texas.

They had a labiaplasty or vaginoplasty somewhere else. Edges that feel wrong. Asymmetry. Over-resection. Scarring. Pain with intimacy. Sometimes a “simple” trim that left them with less tissue than they needed. I am Red Alinsod, MD. Repair is a large part of what I do at Alinsod Institute in Arlington, in the Dallas–Fort Worth area.

Patients fly in from across the U.S. and from overseas. DFW is a hub. My office is about 20 minutes from the airport. We plan the visit so the consult, the awake repair when it fits, and the early recovery can stay in one trip when that is safe.

Teaching has always been in my blood.

When I left California the live preceptorship stayed with a former Fellow. I built Gynflix so the same curriculum could travel.

I am proud of the surgeons who now run their own programs — Alex Bader, Amr Seifeldin, João Brito Jaenisch, Suleyman Eserdag, Sejal Ajmera, and many others.

Almost all revisions I perform are in-office and awake. Patients fly in from all states and dozens of countries. We are so close to DFW Airport.

Office surgery is possible because of how I block, how I stage the repair, and what the remaining anatomy will allow. The Microtumescent Anesthesia and my Pudendo-Levator Block are part of that foundation. Not every revision belongs awake. Not every revision belongs in the office. I sometimes need a surgery center to assure comfort. But it’s rare and available.
