Recurrent thrush, recurrent BV and recurrent UTIs are extremely common, and on this week’s episode @mariaulokomd , urologist and sexual medicine specialist, explains why she puts all three in the same bucket.

The part that is not in the slides is what causes it, and it is not only menopause. Oral birth control changes how your body uses testosterone. So do common acne medications and hair loss medications. If your infections started after one of those, that is not a coincidence, and it does not mean you have to stop taking it. It means the tissue needs the hormones given back to it directly.

Did you know your recurrent UTIs may not be coming from your bladder at all?

I senior authored a new study that @npr just covered nationally. Of 253 women diagnosed with recurrent UTIs, only 15% had a problem limited to the urinary tract. In the vast majority, the root cause was hormonal changes to the vulvar tissue and microbiome, plus pelvic floor dysfunction. The infections were a symptom. The diagnosis stopped one level too early.

If you’ve been on antibiotic after antibiotic and the symptoms keep coming back, you are not crazy. You may simply never have been evaluated for the actual cause.

You cannot diagnose what you were never taught to examine. That is exactly why I built @vulvai.co

The thing I hear most from listeners: you got put on a medication for vulvodynia, nobody explained why, it didn't work, and you walked away thinking that was the end of the road. (And yes, this was also the case for me).

@mariaulokomd puts it like this. Chest pain could be acid reflux, or it could be a heart attack. Those are not remotely the same problem and they do not get the same treatment, so no doctor on earth would hand you an ibuprofen and tell you to call back in six weeks. They'd find out which one it was first. That's called building a differential diagnosis, and most clinicians are never taught to do it for vulvar pain.

So it feels like trial and error because it is. 🥲 This is a foundational episode I've wanted to make for a long time - a whistle stop tour of the medications and interventions available for different types of vulvar pain, and how anyone should actually be choosing between them. ❤️‍🩹

My first EssenceFest was something I’ll never forget.

This was our very first consumer-facing event with VULVAi, and the stories I heard—especially from Black women and those most vulnerable in this system—cut deep.

Story after story of misdiagnosis, mistreatment, and medical neglect.
Women told us how their doctors never gave them options.
How they were forced to make life-altering decisions with little to no information.
And now, years later, they live with the fallout—often in silence.

These stories aren’t rare. They’re everywhere.
Shared at a booth. In a hallway. Randomly in New Orleans.
It reminded me exactly why I do this work.

This week, VULVAi is live at ESSENCEFest in the Health Innovators Hub, and I’m honestly overwhelmed—in the best way.

This dream, 1.5 years in the making, has been the hardest work of my life.
Stepping out with a vision but no roadmap? No small feat.
The lows? Whew. LOW.
But the highs—knowing we have the power to transform care for millions of women—keep me pushing.
