Cassia’s heart is robotic, designed by Anastasia.

Cassia has a genetic disorder caused by broken/mismatched DNA. Genetic disease is rampant in their world; everyone has genetic disease. Cassia's genetic mismatches are unheard of and remain unsolved. Cassia will suffer from neurodegeneration as she ages. She is in the dormant period of this degeneration; she spends her time obsessively trying to find a cure.

Despite this, she is pessimistic about the chances and accepting of the fact that she will die young. Cassia teeters closer to death with each passing day, challenging her fate with her research. If she can't cure herself, the least her research can do is cure others.

Anastasia is significantly more obsessive over the countdown to Cassia's degeneration & death than Cassia herself is. He is unable to accept that she will die, working endlessly to find an out. This obsessiveness becomes significantly worse whenever Cassia dies by electric shock from the very machines Anastasia helped build that were meant to cure her. Anastasia resorts to extremes, desperately converting Cassia into a cyborg to resurrect and keep her alive.

Cassia's heart could not be saved and was replaced by a mechanical heart after briefly being on a pacemaker as part of a bypass.

Anastasia's attempts to cure Cassia become increasingly extreme, even to the extent of Anastasia using himself as a test subject for quality-of-life upgrades to Cassia (such as Cassia's robotic arm. Anastasia had self-amputated his arm to use himself as a subject for man-metal limb fusion.

All flesh rots; only steel is eternal.

Casstasia my lovelies.... Anastasia was quite literally designed for Cassia

There is a overlooming threat of genetic diseases in their universe, everyone has genetic disease but some people just have more niche genetic diseases from broken DNA. There's cures for some of the more common DNA mismatches, but if you have an uncommon mismatch, nothing can be done. Cassia happens to have a unique genetic disorder, that she's unable to solve

Anastasia's childhood was essentially being a patient zero for experimental DNA treatment. Medical companies thought if they could craft the perfect DNA, it could be used to cure everyone else. The project failed, and Anastasia was one of the survivors who had her DNA engineered to make his body so efficient, it ended up circling around and disabled him at the opposite end of the spectrum. Anastasia does not remember this happening to him

While not being made *directly* for Cassia, cassia falls into the group that Anastasia's DNA would have cured. Despite being a failed experiment, Anastasia still ended up devoting himself to attempting to save Cassia
