High hematocrit on TRT does not automatically mean you need to take aspirin.

In this video, Dr. Thomas O’Connor addresses a common assumption among men on testosterone: if TRT raises your red blood cells or hematocrit, should you start taking a daily baby aspirin to reduce the risk of a blood clot?

Dr. O’Connor explains why the answer is more complicated. Testosterone can cause androgen-induced erythrocytosis, but that is not the same condition as polycythemia vera. Treating the two as though they are interchangeable can lead to inappropriate conclusions about aspirin use.

He also explains when low-dose aspirin may have a legitimate cardiovascular role, including in certain people with established coronary artery disease, and why taking aspirin without a clear indication can create its own risks, including gastrointestinal and hemorrhagic bleeding.

The bigger lesson is that men on testosterone need to understand more than just their testosterone level. Hemoglobin, hematocrit, cardiovascular history, blood pressure, clotting risk, iron status, and other health factors all help determine what those laboratory numbers actually mean.

That is exactly why the Testosteronology® Health Portal was created.

Are social media trends pushing men to risk their long-term health in pursuit of the perfect physique?

READ THE FULL ARTICLE:

The growing popularity of “looksmaxxing,” testosterone optimization, anabolic steroids, and experimental peptides is raising serious concerns about men's health.

From cardiovascular complications and infertility to hormonal disruption and the unknown risks of unapproved peptides, the pursuit of greater muscle mass, sexual performance, and physical attractiveness can come at a significant medical cost.

In this new article, the Testosteronology Society® examines how social media is reshaping expectations of modern masculinity, why higher testosterone doesn't necessarily mean better health, and why informed medical care and harm reduction are more important than ever.

Founded by Dr. Thomas O'Connor, the Testosteronology Society® is committed to advancing clinical education on testosterone therapy, anabolic steroid complications, and the evolving challenges facing men's health professionals.

If gynecomastia develops while you are on testosterone, should you try to control it with an aromatase inhibitor, or is surgery the better option once a true mass has formed?

In this video, Dr. Thomas O’Connor responds to a man who has been on approximately 100 mg of testosterone per week for eight years and developed gynecomastia on one side.

Dr. O’Connor explains why the first priority with any new breast mass is making sure it has been properly evaluated rather than simply assuming it is gynecomastia. He then discusses the difference between excess chest fat and a true fibrotic gynecomastia mass, why an experienced gynecomastia surgeon matters, and why he strongly disagrees with keeping men on aromatase inhibitors, tamoxifen, or raloxifene in an attempt to shrink established tissue.

He also explains why aggressively suppressing estrogen can affect the central nervous system, heart, joints, and overall well-being while failing to address a hardened gynecomastia mass.

The larger lesson is that men on testosterone need to understand what is actually happening in their bodies rather than automatically treating every symptom by adding another medication.

That is part of the mission of the Testosteronology® Health Portal: Helping Men on Testosterone Maximize Results and Protect Their Long-Term Health.

No matter who prescribes your TRT, learn what to track, what to watch for, and how testosterone may affect your strength, performance, heart, kidneys, blood pressure, sleep, sexual health, and other areas that can shape your long-term health.

In this video, Dr. Thomas O’Connor explains where enclomiphene and clomiphene may fit for men with low testosterone who are not ready to begin testosterone injections or who want to preserve fertility.

Unlike taking testosterone from outside the body, enclomiphene works through the brain and pituitary system to increase LH and FSH, encouraging the testes to produce more of their own testosterone. Dr. O’Connor explains why laboratory testosterone levels can sometimes rise dramatically while the way a man actually feels does not improve to the same degree.

He also discusses the difference between clomiphene and enclomiphene, their potential role in fertility and post-cycle recovery, and why some men ultimately decide to transition to testosterone therapy.

The discussion also addresses an important limitation: long-term data are limited. Some men may eventually feel less benefit, experience unwanted effects, or simply decide that the treatment is not providing the quality-of-life improvement they expected. Dr. O’Connor is also particularly cautious about this approach in men with significant anxiety or depression based on what he has observed clinically.

In this video, Dr. Thomas O’Connor responds to a 26-year-old man using 350 mg of testosterone along with tadalafil, rosuvastatin, nebivolol, and telmisartan who asks whether he can stay on that regimen as long as his cardiac markers remain favorable.

Dr. O’Connor explains why good laboratory results are only one part of the picture. At 26 years old, he wants to know why this man needs multiple cardiovascular medications, what his blood pressure looks like outside the doctor’s office, and whether the testosterone dose itself is creating unnecessary long-term risk.

The larger point is that “my labs look good” should not become permission to ignore the total amount of androgen exposure or the number of medications needed to keep other health markers under control.

That is the mission of the Testosteronology® Health Portal: Helping Men on Testosterone Maximize Results and Protect Their Long-Term Health.

No matter who prescribes your TRT, Dr. Thomas O’Connor wants men to understand what to track, what to watch for, and how testosterone can affect blood pressure, cardiovascular health, kidney health, sexual function, metabolic health, and the other factors that shape long-term outcomes.

Steroids, peptides, SARMs, and other performance-enhancing drugs are changing faster than healthcare can keep up. Are clinicians prepared to manage the potential health consequences?

A new scientific commentary published in Addiction examines how harm reduction must evolve as more individuals turn to anabolic steroids, experimental peptides, and combinations of performance-enhancing substances. From uncertain product quality and drug stacking to cardiovascular risks and unknown long-term effects, these emerging trends present serious challenges for healthcare professionals.

The Testosteronology Society®, founded by Dr. Thomas O'Connor, M.D., believes that protecting patients requires more than routine blood tests and testosterone monitoring. Clinicians need specialized knowledge, comprehensive medical assessments, and a nonjudgmental approach to patients using testosterone, anabolic steroids, and other enhancement substances.

Our latest article explores the research, the growing clinical challenges, and why specialized medical education is becoming increasingly important.

What are some of the biggest mistakes men encounter when choosing a TRT clinic?

In this series, Dr. Thomas O’Connor breaks down The 5 Top Mistakes of TRT Clinics and explains why proper medical history, qualified clinical oversight, appropriate risk assessment, and ongoing monitoring matter for men using testosterone.

Too often, men begin TRT without fully understanding what should be evaluated before treatment or what needs to be monitored over time. Testosterone therapy can affect far more than testosterone levels alone, including cardiovascular health, blood pressure, blood counts, kidney health, sleep, sexual health, and other important areas.

Dr. Thomas O’Connor’s Testosteronology® Health Portal is designed to help men already on testosterone better understand what is happening inside their bodies, what they should be tracking, and what questions they should be asking.

Retatrutide has moved another major step forward in the race to develop the next generation of obesity treatments.

The newly published TRIUMPH-1 Phase III clinical trial evaluated retatrutide in 2,339 adults with obesity without diabetes. At 80 weeks, mean body-weight reductions reached 17.6% with 4 mg, 23.7% with 9 mg, and 25.0% with 12 mg, compared with 3.9% with placebo. More than half of participants receiving 12 mg achieved at least 25% weight reduction, while 36.9% lost at least 30% of their baseline body weight.

The study also examined important obesity-related conditions. Retatrutide significantly reduced knee osteoarthritis pain and reduced apnea-hypopnea events in participants with obstructive sleep apnea. Improvements were also reported across several cardiometabolic measures. Gastrointestinal adverse events were the most common, particularly during dose escalation.

For clinicians, the significance of TRIUMPH-1 goes well beyond the headline weight-loss numbers. Questions remain regarding long-term safety, body composition, preservation of lean mass, and how retatrutide may ultimately compare with established therapies such as tirzepatide.

The Testosteronology Society® provides clinicians with ongoing education on testosterone therapy, hormone health, obesity medicine, cardiometabolic health, anabolic steroids, and the complex medical issues affecting patients using testosterone and androgens.

Associate Testosteronologist® and Certified Testosteronologist® memberships provide access to clinical education, the Testosteronology® Professional Education Program, clinical reviews, guidelines, presentations, Grand Rounds, live education, and an expanding library of resources created specifically for clinicians.
