ACL Rehab Vlog - Shiv’s at his 4th knee operation 😅

After retearing his ACL again, having the knee cleaned out with another operation to prepare the site & then having it redone this time around, he’s no stranger to rehab.

What’s been incredible to see his how PREHAB has built his strength back alongside muscle bulk allowing him to nail the basics of rehab for ACL really well.

Knee is at zero degrees extension, strength is already coming back on & achieving a straight leg raise with no lag!

How?

We worked hard with Shiv under the guidance of @rush.physio & @andrewthephysio @recoverie.health

The work we do in the prehab phase makes the early post op phase so much better!

Shout out to Mr. Tibrewal for the great surgical work 🙌

ACL rehab doesn’t have to be a lonely road. This is what it looks like when you stop doing it alone

Same injury. Different stages. One room.

If you’re rehabbing an ACL and feeling stuck, frustrated, or isolated, there’s a place for you here. You don’t have to do this on your own.

On Sunday we were proud to host our ACL Collective Workshop and coffee catch up at Recoverie Northwood ☕️

A day dedicated to education, movement and empowering our ACL patients through every stage of rehab.

We wrapped things up with a post-workout coffee and chat, creating space for connection, shared experiences and honest conversations

This workshop was held in support of charity @_icanyoucantoo , and it’s just the beginning. We’ll be running these sessions regularly as we continue to build a stronger ACL community.

Would you like to join in on our next one? taken in Northwood Hills, Northwood by @andrewthephysio

2520 days...
Between 2 international moves, 2 ACL surgeries and throw in a global pandemic for good measure, that's how long it took to finally get back to sport. But 2025 was the year it finally happened.

It has been one of the most difficult things I've ever been through physically, mentally, and emotionally. But it's been one of the most rewarding.

ACL rehab is a battle of attrition, those who persist, prevail.

“Nothing in the world can take the place of Persistence. Talent will not; nothing is more common than unsuccessful men with talent. Genius will not; unrewarded genius is almost a proverb... Persistence and determination alone are omnipotent. The slogan ‘Press On’ has solved and always will solve the problems of the human race.” by @andrewthephysio

The unpopular truth about ACL Injuries & Rehab 🦿

Specificity wins each time - whilst guides or fancy rehab drills may be useful - specific rehab for each person is key!

ACL rehab can be quite lonely - if you haven’t yet built a partnership with your surgeon, physio & coach.

Technically any rehab is team sport & I know that the last 30 days have been nothing short of efforts from everyone involved.

A month since I tore my LCL, ACL, MCL, meniscus - just to add the other knee does not have an ACL either.

I’m walking better, I can play with my kids & take them swimming, drop offs & much more. I haven’t taken a day off work as a physio & biz owner, drove a truck across London, able to even train heavy once again.

This has happened because I’ve built each aspect of the rehab around my lifestyle & using @matt_prehabpro @andrewthephysio to bounce ideas & get working.

The best part of all this is that I have just started to get stronger once again.

I know there’s many who believe one specific way of ACL rehab works - let me tell you it doesn’t. 👊🏽

No top 3 exercises will get you better but the ones that are specific for you. by @andrewthephysio

Finishing touches...

Started the week off with a great assessment with @bibralakephysio to identify what still needs to be addressed as we get into the late stages.

RSI have come up really nicely and some good overall improvements in strength and ankle stiffness so I've gotten the intended result from my last training block.

The main thing left to address is that I'm still a bit slow on my single leg rate or force development and reactivity and and still heavily favoring my right side for landing and deceleration. RFD numbers are primarily going to be performance so keen to get these up just to see how it compliments my on field training, but the deceleration numbers present an injury risk which needs to be addressed before I can consider return to sport.

Today's session:
Warm up bike, box jumps, general mobility.

A1. Single leg altitude landing
A2. Single leg drop jump
B1. Single leg squat jumps
C1. Overspeed lunge repeats
C2. Reactive lateral bounds
D1. Eccentric leg press
D2. Single leg extensions. by @andrewthephysio

What happens when a stoppable force meets an immovable object.

Overcoming isometrics have been a nice staple in my programming the last few months.

Doing short, high effort isos as part of the prep work for a session provides the nervous system a high intensity input to produce better motor unit recruitment, tendon stiffness, and force production. All at a very low relative energy cost.

Doing them at specific angles allows for better transfer to the dynamic work being done during the session. This having good transfer to heavy fast concentric split squats and single leg jumps.

Find a way to set up the exercise of choice where you can gradually ramp up to a max effort push for 3-10 seconds.
If you're sore doing this around 50-70% intensity for 10-40 seconds can work a treat for stimulating local tissue while being a bit gentler. by @andrewthephysio

Training for power is all about intention.

It's as much a nervous system adaptation as it is a local muscular one - training with maximum intent is the best way to signal for your central nervous system to increase high velocity strength.

With this being a global system adaptation, it's more than just jumping. If you aren't training upper body as part of it. You're leaving meat on the bone.

Upper focus day:

Warm up
Med ball rotation scoop
Med ball slam
Isometric bench pin press

A1. Close grip bench press 3*2/2 (rest pause set)
A2. Med ball rotation throw 3*5
A3. Inverted row 3*10
A4. Band assisted push up 3*5
A5. Pogos 3*20

B1. 1 arm DB row 3*3
B2. DB incline bench 3*3.

C1. Pull up 3*3
C2. Barbell push press 3*3

D1. Max effort box jumps

Objective testing can tell us about so much more than just strength.

DSI is one of my favourite things to look at when we're getting to the point where explosive/sport specific movements are getting trained more.

When we use it well it can help fill in the blanks on what an athlete is missing and how to help them make that next step. by @andrewthephysio

May round up:

A birthday for the best boy.

Plenty of knee clients getting into to return to sport stages.

A fun ankle case of having to stay within surgeon restrictions while still progressing a very capable athlete.

Plenty of training and progress on my own rehab.

A few more field sessions while the weather is still allowing it.

Refining online service offering.

Refining my rehab framework for some international education opportunities
.
And a whole lot of hot sauce.

Bring it on June. by @andrewthephysio

While it's been a long road to this point, it feels like we're cooking with gas now.

As comfort has improved and my numbers have started to round a corner, I'm feeling more confident than I have in several months, still playing the long game but feels like we're certainly in the home stretch now.

Putting my methods to test now with a big power training block here with the intention of retesting in about 6 weeks to get some gains on reactive strength, vertical jump, and overall power.

This is week two of this block. Next week volume goes up while maintaining intensity.

Today's session:

Warm up:
Skipping
Short accelerations
Isometric split squat

A1. Box squat. Focus on fast concentric slow eccentric 2 *3,3
A2. Drop jump 2*4
A3. DB jump 2*4
A4. Band assisted jump 2*4
A4. Ankle mobility

B2. Bulgarian split squat 2*3 (5 second eccentric)

C1. Goblet lateral lunge 2*10
C2. Heavy band crab walk 2*10 by @andrewthephysio
