This scenario plays out countless times:

You go to a clinic with low back pain and get assessed.

They send you for imaging after conservative management doesn’t seem to be working.

Imaging shows a disc extrusion (grade 3, shown above) or sequestration (grade 4) and the decision is immediately made to have surgery.

This is all too common a story we hear at ATHLETIC SPINE.

Surgery can often be avoided even with big disc herniations.

This requires a thorough assessment, individualised treatment plan and liaison with your surgeon to make informed decisions throughout your journey.

SPECT CT SCANS ✅

Spect-CT scans are a specialised image technique, combining functional metabolic activity with detailed structural anatomy (Cusi et al., 2013).

This provides us with two important components:

1. The structural changes present, and

2. Regions of higher metabolic activity associated with inflammation, possible sources of pain and so on.

Typically I’d use this type of imaging to confirm suspected diagnosis of facet joint or SIJ joint pathology*

Above is an example of a current client with left SIJ pain, confirming suspicion of degenerative changes and higher metabolic activity, and hence a possible pain source.

This is certainly not a routine imaging procedure, so careful clinical reasoning must be applied.

*please note: this is not the only way I diagnose facet/SIJ issues.

RADICULOPATHY CAN GET BETTER WITHOUT SURGERY ✅

RADICULOPATHY involves strength, reflex and/or sensory changes to a nerve mainly due to compression.

In the first video, the patient shows a clear FOOTDROP on the right side (unable to maintain ankle dorsiflexion).

This was due to L5 nerve root compression from a large L4/5 right paracentral disc herniation.

Flash forward 3 months and the second video shows complete resolution of the FOOTDROP (able to maintain ankle dorsiflexion).

This was achieved through modifications to the clients daily movements/positions, specific nerve “offloading” exercises and a graded strength & conditioning program.

Even in the presence of clear strength deficits, surgery is not immediately the answer for clients with RADICULOPATHY.

JIU JITSU GRADING - 3 STRIPES ADDED ✅

Whilst I’m still white belt, this achievement means more than most would imagine.

After suffering a spinal cord injury and subsequently having neck surgery in 2025 I never thought I’d be able to step back on the mats.

But I trusted my surgeon, my rehab and my coach to look after me. Thank you all beyond words 🙏

To my girls, life will always give you setbacks. But it’s the comeback that matters most.

Whilst there is a LONG way to go, I’m proud of this achievement ❤️

DON’T BE MISLEAD BY THE SCAN ✅

Whilst imaging is an incredible tool for diagnosing patients with spinal pain, imaging is just one component of the diagnostic process; the others being the subjective and objective results.

Here we see an L4/5 central disc herniation (grade 2) with compression of the front aspect of the spinal cord.

While it would be easy for us to conclude that this is the reason for the client’s pain, we note that subjectively her pain was either side of the spine (not central) with NO lower limb referred pain.

Objectively pain was only elicited during spine extensor muscle loading tests, not disc/nerve related ones.

In this instance the subjective and objective results differ from the imaging results and this is purely a muscle overload issue, not a discogenic one.

Remember, imaging is just one component of the diagnostic process, don’t be mislead by it.
