Paralysis is not unconsciousness.

These 11 anesthesiologists deliberately experienced awake paralysis in a controlled experiment. They could communicate and stop. Routine general anesthesia is intended to prevent awareness.

Accidental awareness with recall is uncommon. The ASA estimates 1–2 cases per 1,000 general anesthesia procedures (0.1–0.2%). This includes different recalled experiences, not only paralysis.

Risk varies with the patient, procedure and clinical circumstances.

Emergency, cardiac and obstetric general anesthesia are among higher-risk settings. NAP5 found that about two thirds of analyzed episodes arose during induction or emergence, making transitions especially important.

Three reminders for the OR:
🔷Immobility cannot confirm unconsciousness. Neuromuscular blockers do not provide hypnosis, amnesia or analgesia.

🔷Verify anesthetic delivery. Check medications, IV access, pumps and tubing. Monitor end-tidal anesthetic concentrations during volatile anesthesia; use processed EEG when indicated, including TIVA with neuromuscular blockade. Assess recovery from blockade before extubation.

🔷Listen and follow up. Take reports seriously, document them, investigate and arrange support.
Planned sedation recall, dreams and ordinary recovery memories differ from accidental awareness.

Awareness estimates vary with how patients are questioned and events reported. None predicts an individual patient’s risk.

I highly recommend reading the full article. Reading all the responses from the volunteers is crazy! 👇🏽

It's our favorite reversal now! It's made end of cases so much easier, but we can't just kick back and relax after giving sugammadex.

Yes, it makes extubation criteria easier to be met. However, we still need to be mindful that anaphylaxis or bradycardia can occur with the administration of this drug.

But now you know this! So go wake up those patients and look like a rockstar when you wheel them into PACU.
