A new preclinical study found that repeated blast exposure disrupted mitochondrial function, brain energy metabolism, and neurovascular function, with behavioral changes still present months later.

Sildenafil, better known as Viagra, improved several of those abnormalities, including mitochondrial respiration and fear-related behavior. Notably, researchers still saw benefit when treatment was delayed until 8 weeks after blast exposure.

This is a rat study, not evidence that sildenafil is an established treatment for blast TBI in humans. But it adds to the growing evidence that blast injury may involve prolonged metabolic and vascular dysfunction that could remain targetable well after the initial exposure.

This study mapped backblast exposure from the Carl Gustaf and SMAW using real-world pressure data and computational modeling.

Both systems produced substantial, position-dependent blast loading across the gunner and assistant gunner, with some of the highest pressures reaching the back of the head.

This research does not establish an injury threshold, but it reinforces why repeated shoulder-fired weapon exposure deserves to be understood, tracked, and reduced where possible.
